This is one of the faults with Achievements. Sometimes they glitch up or the system gets disconnected with the achievement requirements.

All you can really do is keep trying. |:

It might be because you detonated a bomb or gas can near one of your teammates. Generally if they get hit by this it doesn't pop up because the damage is so negligible that it's not worth it but I'd think it still counts towards the achievement.

This is probably it. I've hit myself and others occasionally with pipe bombs: it still counts (I think). I've had the same problem, but every time I managed to get someone with shrapnel.

I heard that you have to sacrifice at least 27 virgins soaked in oil to get that ONE achievement that you need to finish the game... have you tested this?

No you wouldn't believe how many parents don't want there 10 year old daughters strapped to an alter and sacrificed.

I guess it will just have to remain one of those mysteries.

An acheivment sometimes does not register until much later, this happened with me on resistance 2 in which my oh so 'precious' trophies weren't rewarded until much later.
