No pic available, but in Raising the Bar (an art book/dev memoir about Half-Life 1 and 2) one of the early monster idea was for a creature that would pin you and basically rape you to death. Obviously it was only conceptual and never even came close to making it in, but the idea is still pretty WTF, especially for a series like Half-Life.

Well there are the floaters in Fallout and Fallout 2.

Also known as the "Sphincter Monster", due to the fact that it looks like a floating vagina with a tail.

What about the Resistance series? The entire reason I took an interest in the first game was because I saw a picture of a Widow Maker in a magazine and immediately fell in love with the design:

For the game you had post just now, it seems to me very generic from what I saw from the other games. Just to see some example, goggle "SMT Satan" and "Maken X" and see the images ^^

As for Resident Evil and Elder Scrolls, some example for extreme weird/taboo/WTF desing?

Yes, but as I said, I want a weird/taboo/wtf design of a character, not about the plot/gameplay of the game.
