It's happened to me before. They'll fix it at the shop, just call it in and wait for your coffin box to come in the mail.

Sounds like the problem my friend has on his PS3 and a problem I had on my PS2. Your Disc Reader is completly Schnozzled. You need repairs from Microsoft/shop.
