All 3 COD MW games, including veteran for all spec ops missions and at least 30 rounds survived on every map in survival.
All 3 PS" Sly games (Thieves in Time isn't out here yet).

Bad Company 2 I have every trophy and I'm pretty near the max rank and beat the campaign 100%.

Killzone 2 and 3, Resistance 3, both Infamous games I've cleared apart from the odd weird trophy. Final Fantasy X including Penance unguided (-shudder-).

Devil May CRy 3 - every difficulty with eveyrhting top ranks. How you ask? Learn royal guard.

Both BioShock's. Umm... Sonic 1, Sonic 2, Sonic 3, Sonic & Knuckles, Sonic Adventure, Sonic Heroes, Sonic Colours, Metal Gear Solid 2, 3 and 4, Kingdom Hearts 2.

RESIDENT EVIL 4 - countless times, to the point where I beat the game on professional with a knife, a magnum bullet and an rpg.

Edit: Assassins Creed 2, TWEWY. Can't think of anymore right now but I've played far too many games. I don't generally care about 100% though.

Very, very few but let me see if I can get some of these out...

Dead Rising 2: Case Zero (though admittedly, that was really easy)
The Legend of Zelda (not counting the Master/Secondary quest: NES version)
Link's Awakening DX (Ton of fun and one of the earliest games I completed)
Zelda 2: Adventure of Link (most satisfying game I've beaten)
Legend of Zelda: Ocarina of Time (tons of fun)
Legend of Zelda: Majora's Mask (this one took me a while)
Mario's Picross (I've beaten every single board on this.)
Metroid (Original NES version: collected every power up and got the super-sexy-bikini ending)
Stacking (I loved this game! It was very short and easy but intensely charming)
TMNT 89' Arcade (it's a beat-em-up...not a lot to miss)
Punch-Time Explosion (3DS...super easy and not entirely worth it)
Super Hero Squad: Infinity Gauntlet (3DS and just like Punch-Time Explosion in that it was easy and not worth it)
Pokemon Snap (pretty easy)
Dokney Kong Country (one of my favorite SNES games)
Final Fantasy Mystic Quest (it's really easy...I liked it but most other FF fans don't)
Mighty Morphin' Power Rangers (SNES beat-em-up)
Beavis and Butt-Head (SNES beat-em-up/adventure: pretty easy)
Super Castlevania IV (a very straight-forward game: pretty easy in terms of completion)
TMNT: Turtles in Time (SNES beat-em-up...easy for completion)
Celebrity Deathmatch (Xbox fighting game: easy but not worth it)

I can't think of any others at the moment...a lot of these are kinda old and easy in terms of completion since there isn't anything you really need to collect.

I got to 100% on Pokemon Blue - at a time when there were only 150 Pokemon. Still counts, though :p I mean I got the "You caught them all!" diploma from the (Cerulean?) scientist at the lab.

Hmmm I made it to 101%, 102% and 103% on all three Donkey Kong Countries on the SNES. I also got 100% on Super Mario World, Yoshi's Island, Kirby's Dreamland 2: Shooting Star, and a whole bunch of other SNES games. The Mario/DK/Kirby games were the only ones that chronicled your percentage, though, mostly because they were some of the few that would let you beat the game without completing EVERYTHING in them. So you had that for incentive.

I also 100'd Spyro 2: Ripto's Rage several, several times on the PS1.

On the PS2 the only games I got 100% of are Kingdom Hearts (fuck you, Seph), Gun, Mercenaries: Playground of Destruction, Odin Sphere and Okami. And I came this close to Kingdom Hearts II but I just couldn't bother with some of the underworld cups.

The original 3 Spyro games for the PS1.

Recently I replayed the original and got through it 100% in less than a day without missing a beat.

Good times. Gooooooooooooood times.

A lot of games, most of which I don't really recall.

My most recent 100% was Catherine, which is one of the most balls-hard 100%'s I've ever done and I enjoyed it from start to finish. Gold'ing all the stages on Hard, the huge pain in the ass that is Rapunzel's 128 Stages followed by the extreme difficulty of the Babel Stages was something that took me well over 50-60 hours to do.

I even did all the Babel stages in both Solo and Pairs (playing with both controllers on my own of course, you think I have friends?), which is completely unnecessary for 100% but just something I felt obligated to do.

These days, I've been trying to improve my high-score for the Babel levels. And at this point I actually have the #1 European Xbox 360 score for Solo Axis-Mundi. If that ain't 100% completion, I don't know what is.

Persona 4 wasn't a hard 100%, but a long one because getting the Persona Compendium filled out just takes a long time with the wiki more than any relevant skill. Most of the personas are useless anyway, you quickly fall into the combination you like. It was a 100% I couldn't be arsed to repeat with Persona 3 Portable which I played later on.

On Xbox Live? None, actually. I tend to run through and stomp the game on the hardest difficulty possible right out the gate. After that, I tend to get bored with the game unless it's genuinely got my interest.

Now, I've beaten every campaign and every solo map on Heroes of Might and Magic III, and I had a level 99 Hardcore Barb on Battle.net back in the day...
