From the top of my head from the PS1 + PS2 era:
Crash Bandicoot 2
Crash Bandicoot 3
CTR
Crash Bash
Spyro 1, 2 and 3
Croc: Legend of the Gobbos
Killzone
GTA: 3, Vice City + San Andreas

Gameboy Colour games:
Pokemon Blue
Pokemon Silver

Xbox 360 games:
Assassin's Creed 1 + 2
Batman: Arkham Asylum
Beyond Good & Evil HD
Call of Duty 4: Modern Warfare
Crysis
King Kong
Lego Harry Potter 1+2
Lego Indiana Jones
Mass Effect 2
Minecraft
Naruto Ultimate Ninja Storm 2
Elder Scrolls 4: Oblivion
Spec Ops: The Line

When I was a kid I didn't have that many games so I just played the shit out of what I did have. Now I'm older I don't really go for completion unless I really like the game and it isn't an utter pain in the arsehole to 100%. Time is also an issue now.

Fire Emblem GBA (Oh god I want awakening to come out now. NOW)
1. Which GBA one, the first?
2. Uhh... Awakening is out. Don't get it unless you have nothing to do for a week, it's Sacred Stones with everything better.

Also, I remembered a few:
Fire Emblem: Sacred Stones (okay, maybe not all the converstaions, but I got most, i beat the game like 5 times)
DK64 (10 hours were devoted to those two Coins, I sware...)
1st Gen Pokemon: I caught all 150, and have since lost my proof... >.>

Awakening doesn't come out for over a month here in the EU. It's depressing. I meant the first one, but I'm starting sacred stones now for the first time to satiate my hunger.

I was a completionist before I stopped caring and now play my Wii lol.

100% in Mass Effect 2 and 3, Skyrim and Oblivion,Fallout 3 and New Vegas, Fary Cry 3, Saw 2, Condemned 1 and 2, Sims 3, Resident Evil 4 and 5, Tony Hawk HD, Saints Row 3, Prototype 1 and 2, Arkham Asylum, Harm's Way, Halo Anniversary, Lego Batman, Bioshock 1, Xmen Wolverine, probably more.

Almost had Arkham City, Modern Warfare 2, Hitman Absolution, and more but DLC is a ***** for this kind of thing.

Soon to be Walking Dead, only one more chapter. I had Dishonored until that shit DLC came out so I traded it in.

I've 100% in a bunch of games I have played over the years. It'd be hard to name them all. Most recently I got 100% in Persona 4. Finished all the S.Links, finished the compendium and beat Margaret :D

EDIT: Once I start a new game of Tomb Raider I'll finish my 100% of the single player campaign. Missed three achievements on my first playthrough.

That I can remember: Marvel Ultimate Alliance 2, GTA 4, Dynasty Warriors 7 Empires and Dragon Age: Origins.

Oh and Pokemon Blue, that was my first 100% complete game.

For me my 100% completions are:

L.A. Noire
Assassins Creed I
Assassins Creed II
Fez
Limbo
Uno
Death Spank
Death Spank 2
CSI: Hard Evidence (kinda an easy one, I know)
Battlefield 1943
Lego Lord of the Rings
Lego Indiana Jones
Lego Batman
Lego Indiana Jones 2
Lego Harry Potter 1
Lego Harry Potter 2
Skyrim
Crackdown (I'm missing one, but it is forever unobtainable due to a MP glitch)
Dragon Age 2 (I'm actually missing one here actually, again due to a bug)

I also have the following with 100% on any singe player achievements and am only missing 3 or less Multiplayer Ones. (I HATE MP achievements, as the vast majority of them are just designed to entice you to grind on the MP for as long as posible to extend the life of the game. This is something I advocate *heavily* against on titles I work on.)

Assassins Creed II: Brotherhood
Assassins Creed II: Revelations
Assassins Creed III
Battlefield Bad Company 2

Lastly, these are the ones I need only 1 or 2 more to get
Dishonored
Arkham City
Shadow Complex
! vs 100 (Sadly I'll never be able to get that last 1 now.. :/ )
Arkham Asylum
Call of Duty Classic
Halo: Reach

In general I'm not a completionist on all things, as I have played about 210 games on the 360, and only completed what's above. However I do like to try to keep my total average of completion to as close as 50% as possible. (Right now it's down around 41, which is something I am actively working to bring up.)

D: I don't remember the last time I played a game with a completion percentage... I know I have all of the Spyros and Kingdom Hearts 100%-ed though, heehee~ Oh! And FFX-2. That one had a percentage thingy. I'm sure I have more games like that, but for the life of me I can't think of any of them...

I'm not really one to care much about *completing* a game, so long as I beat it, but it seems to happen regardless of my interest after messing around on the same save/account for so many years.

I've never made an 'achievement' on a game before, though. I don't have those consoles.

Easier games... or at least ones that don't challenge me that much.

The World Ends With You, Little King's Story, Legend of Zelda TP, Okami (kinda), Half Minute Hero, KH BbS etc.

Oh, and Kingdom Hearts Days. Well, it was like 98% done and then I deleted my data. I refuse to play that game ever again...

I think a lot if it is how you define 100% complete. Also, some games have tons of extras to do, making completion harder.

All 3 COD MW games, including veteran for all spec ops missions and at least 30 rounds survived on every map in survival.
All 3 PS" Sly games (Thieves in Time isn't out here yet).

Bad Company 2 I have every trophy and I'm pretty near the max rank and beat the campaign 100%.

Killzone 2 and 3, Resistance 3, both Infamous games I've cleared apart from the odd weird trophy. Final Fantasy X including Penance unguided (-shudder-).

Devil May CRy 3 - every difficulty with eveyrhting top ranks. How you ask? Learn royal guard.

Both BioShock's. Umm... Sonic 1, Sonic 2, Sonic 3, Sonic & Knuckles, Sonic Adventure, Sonic Heroes, Sonic Colours, Metal Gear Solid 2, 3 and 4, Kingdom Hearts 2.

RESIDENT EVIL 4 - countless times, to the point where I beat the game on professional with a knife, a magnum bullet and an rpg.

Edit: Assassins Creed 2, TWEWY. Can't think of anymore right now but I've played far too many games. I don't generally care about 100% though.

Very, very few but let me see if I can get some of these out...

Dead Rising 2: Case Zero (though admittedly, that was really easy)
The Legend of Zelda (not counting the Master/Secondary quest: NES version)
Link's Awakening DX (Ton of fun and one of the earliest games I completed)
Zelda 2: Adventure of Link (most satisfying game I've beaten)
Legend of Zelda: Ocarina of Time (tons of fun)
Legend of Zelda: Majora's Mask (this one took me a while)
Mario's Picross (I've beaten every single board on this.)
Metroid (Original NES version: collected every power up and got the super-sexy-bikini ending)
Stacking (I loved this game! It was very short and easy but intensely charming)
TMNT 89' Arcade (it's a beat-em-up...not a lot to miss)
Punch-Time Explosion (3DS...super easy and not entirely worth it)
Super Hero Squad: Infinity Gauntlet (3DS and just like Punch-Time Explosion in that it was easy and not worth it)
Pokemon Snap (pretty easy)
Dokney Kong Country (one of my favorite SNES games)
Final Fantasy Mystic Quest (it's really easy...I liked it but most other FF fans don't)
Mighty Morphin' Power Rangers (SNES beat-em-up)
Beavis and Butt-Head (SNES beat-em-up/adventure: pretty easy)
Super Castlevania IV (a very straight-forward game: pretty easy in terms of completion)
TMNT: Turtles in Time (SNES beat-em-up...easy for completion)
Celebrity Deathmatch (Xbox fighting game: easy but not worth it)

I can't think of any others at the moment...a lot of these are kinda old and easy in terms of completion since there isn't anything you really need to collect.

I got to 100% on Pokemon Blue - at a time when there were only 150 Pokemon. Still counts, though :p I mean I got the "You caught them all!" diploma from the (Cerulean?) scientist at the lab.

Hmmm I made it to 101%, 102% and 103% on all three Donkey Kong Countries on the SNES. I also got 100% on Super Mario World, Yoshi's Island, Kirby's Dreamland 2: Shooting Star, and a whole bunch of other SNES games. The Mario/DK/Kirby games were the only ones that chronicled your percentage, though, mostly because they were some of the few that would let you beat the game without completing EVERYTHING in them. So you had that for incentive.

I also 100'd Spyro 2: Ripto's Rage several, several times on the PS1.

On the PS2 the only games I got 100% of are Kingdom Hearts (fuck you, Seph), Gun, Mercenaries: Playground of Destruction, Odin Sphere and Okami. And I came this close to Kingdom Hearts II but I just couldn't bother with some of the underworld cups.
