The hardest difficulty on Far Cry 2. Every encounter was dreaded, in the end I had to drop it by a level. The difference was massive.
Every encounter was dreaded? Every 2 seconds you're getting run off the road by lunitics. I swear that game Yahtzee got that game nearly spot on, they will drop whatever their doing to run your ass off the road. (though it is a fun game)

Every driving mission that I completed. I suck so much at driving in games, it's one of the very few reasons for me to quit a game.

And Sephiroth in both Kingdom Hearts games. Man, what a fight!

Probably getting a pants explosion (planting a live grenade in someone's inventory) without getting caught in Fallout 3. 85 in Sneak and I still can't do it without a Stealth Boy or the Chinese Stealth Suit.

Maybe this will qualify, but I personally thought finishing Gears of War 2 on the most insane difficulty level available (solo) was quite something. Once I learned that if I so much as showed a nosehair out of cover at any point, I'd be shot down before I knew where I was...:p

:)
Did you find Dom to be an absolute game breaker on insane? Man, I had so many game overs because of his stupid AI ass running out into the line of fire and having to go on a suicide mission to revive him.

Also the reavers/satt comm section in the final act was just horrific. It took me at least 20 tries to beat that.

Beating Typhon, Bane of the Gods, in Titan Quest on Epic difficulty (nr.2 hardest dif) after bout 9 tries. it was really epic to see his awesome 4-handed, bald, spiky body dissolve into bones before all to the floor.

If anyone else here has beaten the turd filled asspipe of a game Red Ninja then you get a pat on the back from me. That game was truly infuriatingly hard because it was just fucked.

The last boss in Warrior Within was a tough cookie too. Kaileena I mean... the dahaka was easy compared to her.

Completing 5 light gun games in one day (in an arcade) with only one credit for each in succession (I am the almighty badass of space and time when it comes to lightguns). If you need titles: Ghost Squad, House of the Dead 3, Time Crisis 3, Silent Scope and House of the Dead 4 (I'm a huge HOTD fan).

That may not be the hardest thing anyone on here has achieved but I still think it's pretty damn impressive.

You have no idea (well, okay you clearly do)! The worst situation was during the section where you have to clear the area for the rest of the COGs to land in the hollow. At that point, Dom seemed to an acquire an even more suicidal tendency than usual, heading right into the midst of the enemies, which always resulted in him crawling around shouting to be revived. At that point, being down to one man made it hard to deal with the locust.

It wasn't long before I was wacked with a mortar and had to start again, only for Dom to repeat his kamikaze run, over and over again!

TTFaF on Expert
Zero's second mission on GTA:SA
Omega, Yizamat, Hellwyrm on FF XII (Even though I had cheats on, they were still kicking my ass. I would hate to imagine them w/out cheats)

I would say beating Kaizo Mario...but I just watched someone beat that...on youtube. And of course I still yelled at the screen for cheating!

I'm going to be slighty different and say the hardest thing I've done in a game was having to choose between Ashley and Kalenko in Mass Effect.

Edit; for the record, I went with Ashley.

The weapon master mode of Soulcalibur 2 has one battle against the Hellfire enemy that's almost impossible without unlocking the ultimate weapons (which I had not). He has about double the normal HP, max AI skill and uses 3 different move sets during the fight. I beat it on my 74th attempt.

Grand Theft Auto: San Andreas Flight School: It's easier flying a real plane I bet.
Ghost Recon (the original): Mission 3, with the advancing tanks and NATO forces that need protection.

World in Conflict. Veteran difficulty. Period.

Well, except for the last two levels. The second to last is impossible.

Killing Ruby goddamned Weapon on FF7, with only Vincent left, Cloud was ejected and Barret was dead. Well that was most frustrating anyway. It's a toss up between that and killing Omega Weapon on FF8, at level 100, the boss is just sick.

As stupid as this sounds some of the challenges in Jak and Daxter 2&3 particularly escaping from the water-slum area and driving the damn rocket. I swear there were some challenges in those games that the easiest way to beat them was to wait for it to become muscle memory.
oh my god, thank you for that. so, so true. the escaping the water-slum area is just ridiculous, i've re-played the games a coupla times and it NEVER gets any easier. the rocket was less of a pain, but still definitely one. weirdly enough the final boss battle in jak 3 is a piece of piss.

i've read jecht, which is right, took me f'n ages (probably about 10 hours) to train my team up enough to beat him. i was close to giving up on that game, and if it weren't for my love of the story, i wouldnt have bothered.

also yeah in SA the flight school and all the zero missions (except the last one) are brick.

Either beating Guitar Hero 3 on Expert (screw you Lou) or completing the Nintendo Gallery in LoZ Wind Waker
