Pretty much spiders, scary images (like e.g. freaky human), inhuman voices or unnatural creature sounds and monsters from films that try to kill you.

Also, ghosts and etherials aren't my favorite species of things to stalk me.

Spiders, some insects, deep water, some social situations, repeated holes across a material(especially flesh after a creepy image I saw on here once), zombies, the dark and finally house fires and other such natural disasters.

I think that's about it. I am easily spooked really. Given enough time trying to get to sleep I normally end up terrifying myself with things that I have created in my imagination that then hold me further from sleep.
It gets quite annoying.

It's kind of odd, but deep water IN GAMES. I don't have hydrophobia in general and am not afraid of swimming in real life, it's just deep water in games, because when I was little I got incredibly freaked out by the piranhas in serious sam. ;d Oh, and maybe having too much responsibilities and unable to cope.
