I personally will never forget the intro to dead space when you first see the necromorphs. They bust into the room, murder the two guys and you have to run for your life, weaponless to an elevator. The music and the atmosphere adds to the fear.

I also love the warthog driving scenes at the end of Halo 1 and 3

The final warthog race in the 1st halo got my blood pumping the 1st time. And the one in the 3rd half way recreated the experience.

Indigo Prophecy had a pretty sweet opening

I still remember the first time I beat the real Andros in Star Fox 64 and had to fly out with fire chasing you.

CoD 1's Russian campaign was awesome. From the opening in Stalingrad where you're running towards German machine gun nests with just five bullets but no gun to the last level where you're huddled in a room on the top floor with an MP40 aimed straight at the door when all of sudden you hear the Russian army has arrived to save your ass.

The knife fight between Leon S. Kennedy and Jack Krauser in Resident Evil 4.

I've never seen a more stylish and glamorous knife fight before, in a video game nonetheless!

STYLISH!

The part in Crysis where you get out of the VTOL on the hill at the break of dawn and the Koreans are shelling the position with everything they've got. Say about the game what you want, but that part was pure awesome.

I'm going to be ARMA fanboy again and say any ARMA firefight. Especially those with vehicles.
As for scripted moments, Call Of Duty 3 first level.

I very, very much agree.

Not so much a sequence, but a magic moment nonetheless: Pyramid Heads infamous "love scene".

"Wait, what is he doing with that... Oh no, he couldn't! Oh my god!" *starring at the scene in disbelief, disgust and primeval fear*

Indigo Prophecy had a lot of intense sequences, i quite liked them. Most of the QTEs were well done. Also, i agree with CoD 1, that 5 bullet part in the Russian Campaign was awesome.

Also, from True Crime: streets of L.A., The Good Cop final boss fight was really intense. It was quite hard, but the music suited it perfectly. I still remember how i and him both had very low hp, and for the killing blow i threw something into his face, stunning him shortly, while i kicked him into a fire. That was really intense.

Any of the times I got chased by that damn Dahaka in Prince of Persia: Warrior Within. Once the color washes out of the screen, you know shit's about to get intense.

Jumping onto Avion, the 5th colossus, as he proceeds to fly over the lake trying to shake you off while you flail around, hold onto his fur, and try not to plummet into the water below before you stab the bird. Extra points if you manage to jump from one wing to another while he's rolling over!

Also, knocking down strider after strider with guided missiles in the big battle in front of the museum. In HL2, obviously.

Currently it's the part in LIMBO where you SPOILERS( just in case this counts as one) have to jump over the blades while the level is flipping itself upside down.

The most epic video game sequence for me wasn't on a console. It was on a flash rpg(GASP!! blasphemy...). What happened was I was playing some blatant rip off of the already ripped off Star Trek Universe and about 200 tiny ships were flying straight at me. I didn't care much, I had the best ship and the best weapons. It turns out they were dragging a painfully slow almost exact replica of a Deathstar from Star Wars. Panicking because of the sheer size, I took off in a hurry and accidentally wasted my entire supply of antimatter jumping around the universe for no reason. Meaning I had to pilot a painfully slow ship against a painfully slow enemy in a painfully slow chase scene that threatened to destroy 4 months of work, as there was no quick reload and death means you lose everything...
