Well, the best ones to me is probably the Xbox 360 cases and Limited Edition ones, as long as it's not one of the cases that has clamps that hold your game so hard it breaks. Bioshock had a metal'ish Limited Edition casing that looks awesome but murders discs (I still have the casing because it looks epic). The color is nice and fits in with the rest of my gaming gear.
Worst ones... I don't know... think I'll throw my vote to the PS1 cases because from what I can recall, none of the games I own from them have a complete casing, most of them have been broken from just opening the games.

Plus, there's also the disappointment these days of opening up a new game and seeing that "manual" in there...that consists of a single folded sheet of paper that contains mostly legal info (maybe info on controls if you're lucky).
I don't know what you do with your cases that needs the plastic thicker and more expensive, but I do agree on the manual part. I love manuals, and I hate seeing them shrinked into legal info with nothing interesting what-so-ever.
