Do villain protagonists count? Because I really have to mention Waldo Trumbull from the movie "The Comedy of Terrors".

Mr. Trumbull, played with great relish by Vincent Price, is the thoroughly corrupt and violently unpleasant proprietor of a small undertaking business. He is constantly on the edge of bankruptcy - very much thanks to his habit of binge drinking whenever he gets his hands on any money.

In between abusing his assistant (Peter Lorre) and trying to poison his father-in-law (BORIS KARLOFF!), Trumbull manages to keep his business afloat by stealthily murdering people at night so he can bury them in the morning. This comes back to bite him HARD when it turns out that his next "client" (Basil Rathbone) suffers from a condition that sometimes causes him to enter a temporary coma that is very nearly indistinguishable from death...

Vincent Price is an absolute joy to watch in this movie. The character of Trumbull is so over-the-top, cartoonishly evil that most actors would have just played him for cheap laughs, but Price (who WAS practically born for this kind of gig) plays the part as straight as possible - and actually manages to make Trumbull both spine-chilling AND comical!

Magneto from comics is my favourite villain, he has cool powers, a fantastic costume design and a motivation that is underestandable and at times make you want to side with him.

Patrick Bateman from American Psycho, book and film (by which I mean: I liked both for different reasons)

Also, Darth Vader, Alex from A Clockwork Orange (book and film, again), Kefka from FF6(III), Al Swearengen the bartender from Deadwood (first season only, please), Obrien from 1984, Keyser Soze from The Usual Suspects. In no particular order.

Saint Dane from the Pendragon Series because he is more along the lines of the dark side of humanity/whatever territory they were in.

That would be Chelsea from Scott Sigler's "Contagious."
Never again will I hear the term "God's love" without a slight shiver.

Umbridge from Harry Potter.

Hey, I may get some flak for this, but Umbridge was written so well that I actually developed a truly deep-seeded hatred for her. When she appears onscreen in the movie adaptations I just sit there and grit my teeth. Grrrrrr. It is made evident, not just through stories, but actual actions that you witness her doing that make you hate her. I agree that Loghain (DAo) is an excellent villan and Bioware(ofcourse) writes him excellently, but a lot of the major dicky stuff he does happens off screen. For umbridge, you're there for all of it. And that gets to you.
I completely agree with you. She's created in a way that you can't help but hate her. Every time she gives that little giggle in the movie you just want to slap her!

Probably either Pinhead (Hellraiser) or Randall Flagg (Stephen King's works).

Randall Flagg for somewhat the same reason. He's just so cheerily evil. He'll cut throats, crucify dissenters, convince the lost and lonely to do the same, threaten and almost succeed with nuclear holocaust, conquer entire countries, all with a smile on his face and a song in his heart. He's totally unrepentant... and it's just skin-crawlingly creepy. And if you kill him, he just regenerates elsewhere and elsewhen.
I have a problem with people calling Randall Flagg a "villain"...incoming Dark Tower 7 spoilers...

...he was a servant of Gan (The Dark Tower), as revealed in Wolves of the Calla. And if you look at his actions in The Stand he wasn't technically evil per say, just fulfilled the role of "the evil, revelation" character. Technically he was God's Necromancer in a sense. Not to mention he seems to know ahead of time (palaver at the end of book 1) that Roland is in an infinite timeloop everytime he reaches the top. Not to mention there's the theory that he's the reincarnation of Merlin and Roland is Arthur. Hence the whole "leading him" thing. Making him follow him across the desert? Lud, the train? Making Roland realize that through the death of his mother that everyone he comes across dies? He's very behind the scenes with everything. Not to mention there's that theory that the Wizard and Glass characters are a reincarnation of the original King Arthur cast.

I am just saying, he's a deeper character than most people give him credit for.

O'Brien from 1984.
It takes a special kind of evil to work on something that long, just so you can set the guy up to break him down.

Game: Revolver Ocelot, I just think he's cool, and he's probably one of the only characters in the series who knows what the hell is going on.

Movie: Bill from Kill Bill. I don't know why, but there was just something about this guy that made me think "badass." Too bad he doesn't get that much screen time.

Book: Darth Bane, partly because he's awesome, and partly because since he's the protagonist of three books, we really get to know what kind of guy he is.

Honorable Mentions go to:
Game: Ghaleon from the Lunar series. He's calm, cool, and more of a manipulator than even the god of chaos could have predicted.

Movie: I'll let you know when I think of one.

Book: Lord Voldemort for giving me nightmares as a kid.

Albert Wesker.

Unless John Stamos has voiced a villain, with his sexy voice.
In which case, whoever John Stamos voiced.

EDIT: It's one of each? Um, okay then...

Movie: Gideon Gordon Graves, just because Scott Pilgrim is my favorite movie xD

Game: The aforementioned Albert Wesker

Book: Mama Elena from Like Water for Chocolate (Originally Como agua para chocolate).

Hard to say who my absolute most favorite villain in any one of the three categories are because of the sheer quantity of how many have left an unshakable impression on me.

However, I will say that Mal from Inception was an interesting case. Her unpredictable reactions, disarming expressions, and calm disposition made for a very memorable (if slightly disturbing) individual!

Grues. I am likely to be eaten by them.

Also Shira from "Blade of the Immortal". So deliciously fucking evil.
