My favorite would be the true murderer from Persona 4.

Then again I love most villains whose motivation is simply "because I could." So that also puts Hazama from Blazblue and Kefka up there.

Myrkul from Neverwinter Nights 2, Mask of the Betrayer
Cold, cool, and one who appreciates irony, Myrkul is a dark god who subjected many to a horrid curse that devoured them from within. It not only destroyed their lives but also their very existence. Why? So he could live on, even in death. As long as the curse remained, he could not be fully destroyed. Fully honest, he rebukes your half-celestial companion, and laughs at her naiveté towards the world and the planes. He causes your most strongly willed companion to begin to doubt her ?crusade? against the Wall of the Faithless, as he provides the truth: the first crusade was not fought over ?justice?, but rather selfish love. He moves on to the rest of your companions, stating the truth, in an attempt to cause them anguish. After your interrogation, you can choose to use the curse he caused to consume his soul, or to destroy him utterly by sending him to his eternal rest, something that is nearly impossible in FR setting (As even death is something you can get better from). If you have the specific companion who is the combination of a thousand damned souls, you can have them devour Myrkul, and enslave a god. His last words are ?A final irony, even in this?. Team Rocket From Pokemon. So funny, they fail at all they do. By normal logic, the sheer number of times they have ?blasted off again?, they should be dead. They are persistent, you have to give them that.

I would restrict it to the Umbridge from the books. Movie Umbridge isn't really all that evil, just kind of irritating. Without reading the books I don't think people would find her to be the evil cruel ***** that the was created to be.

Book Umbridge is pretty damn horrible though, she gets one of my votes, not just for her role in Order of the Phoenix but also in Deathly Hallows where she reaches her full evil potential. I hated her more after finishing her part in Deathly Hallows than I did after Order of the Phoenix.

Clyde Shelton from the movie "Law Abiding Citizen".

This is the first movie, (or at the least the only one i can remember) that i want the bad guy to win after about 10 minutes into the movie.

My favorite villain...EVER!

This is an awesome movie... WATCH IT! I command thee!

So many good ones, I'll just name off the top of my head.

Movies - Dracula from Francis Ford Coppula's Bram Stoker's Dracula. He's so beautifully evil. Some find him justified or pitiable, but I think he's in the end an evil monster, but you can feel his evil.

Books - Well I'm just finishing Fall of Hyperion from the Hyperion series by Dan Simmons. So I'll say the Shrike. The books essentially are all about the Shrike. Whether or not it's a villain is debatable, but it certainly kills a lot of innocent people (whether or not they're really innocent is again, debatable).

Games - Just finished DA2 for the second time and Knight-Commander Meredith is a pretty well realized villain, especially if you read the codexes and ask about her back story.

ps - Loghain is much more misguided (though maybe not actually) than evil and certainly not on a power trip. He freed Fereldan from the Orlesian occupation with the late King Maric. He hates the Grey Wardens because the major Grey Warden presence nearest Fereldan is in Orlais. His greatest fear is that after the Grey Wardens defeat the Blight that the Orlesian Grey Wardens will reconquer Fereldan, which is a real possibility. But he did betray his king, and there's no justification for that (unless your revolution is successful (ie America, et al)).
