I made plenty of fake IDs for age reasons when I was young. Always use your real name, and just modify the year of your birthday, anything else is likely to get you caught.

For an identity changer, a very common and easy to remember name like David Black. No one would think twice, you won't forget it.

I've always been told to change your last name, but keep your first name (if you're in a tight knit area, people could recognize the last name.

As such, it's Jack Stark.
