I generally don't like underwater levels either. And the thing is that I could enjoy them is developers weren't so dead-set on making the character/vehicle I have to move control like shit then they could be great, since quite a few of them are beautiful to look at.

Next up would be areas that are so dark you can't see more than a few feet in front of your face and sewer levels because they almost always end up being a maze of somekind and I hate that. And Dark Souls is guilty of both of these things (The Depths and Blightown). And Demon's Souls had some of that too (Valley of Defilement).

Anything that looks really open to exploration at first and turns out to be incredibly linear. Jungle settings in particular, it totally sucks when what initially seems like a wide-open environment teeming with life for you to explore is essentially a reskinned corridor.
Seconded. Really kills the immersion, too!

That said, I don't particularly dislike any specific KIND of environment that gets thrown at me - snow, lava, desert, quaint village, ancient puzzle maze, bayou, mountain, industrial puzzle maze, modern city, pirate ship, sewer puzzle maze - just as long as it's done well! I always appreciate it when a game offers plenty of variety. The more different environments, the better, I say!

I'm fine with most levels, but I don't like massive, samey levels that aren't just large, but also boring. ie. Lost Izalith.

Deep and dark underwater levels. Just swimming about in a 2d well lit water land? I'm cool with that. Dark waters and going deeper by the second? Bad stuff and I am probable long since freaking out. It is a fact the darker/murkier the water, the quicker you will see giant glowing eyes swimming after you...so basically all the things my video game nightmares are made of.

In a close second with a super slippy ice levels. Ice levels with a small amount of slide, where you feel the least bit of control? Annoying but still decent. However some games just need to make ice so slick it feels like a horrifying torture trying to slide away from the most certainly there bottomless pits.
