I'm not quite sure where people are getting it from that it's because of it's nature. Honestly, the only extreme things I've seen are to do with advertising, such as the cover art and trailers. The actual game doesn't even seem that explicit, nothing we haven't seen in other games anyway. They'd just need to tone the advertising down for the US, including changing the coverart.
The other option is skipping us and releasing it in europe which isn't as bad for nudity or sex. Unfortunately that isn't an option considering ATLUS doesn't have a base in europe. I suppose a different group could do it though, perhaps.

Someone already hit the nail in the head with the reason why it's surprising. But yeah, this makes me both saddened and annoyed that we here in the US are sexually immature and needs to either cry foul or illegal for the most dumbest of things.

If you follow japanese rpg developers, then you won't find it surprising at all. It isn't the first japanese game to be not brought over. I was surprised by The Last Story, as I too expected bringing it over to be the norm. But now, I don't find it surprising at all. Not that I think it makes sense. Catherine has already had a tonne of publicity which will have helped sell it greatly.
