Now I remember a great plot twist. In Baten Kaitos:

I was shocked when I found out that Kalas is actually a bad guy but I knew that Melody (is that her real name? I can't remember) is a villain.

Some of what I'm about to say has been said...but I can't help it:

KOTOR: The 'Past' reveal. Crazy. I didn't see it coming.

Baldur's Gate:
- Imoen being a Bhaal Spawn too. No WONDER there was no romance story with her! Creepy.
- Seravok and Kovares being one and the same. That dirty jerk.
- Khalid dying. I LOVED Khalid, and when I found his broken body (Granted, very early in the game) I was like 'awwwww dang'.
- Yoshimo betraying you. He was so freaking handy, too.
- Edwin finally getting his book of great power...and turning into a woman. Freaking hilarious.

BAD plot twists:
KOTOR 2: The old lady being bad. Who didn't see that coming?
And I sadly have to agree with the Fallout 3 ending. Pretty piss poor explanations of why Fawkes couldn't do it. It would have been better if there was some explosion and every died in the facility...BUT you, making you the only one who could have done it..instead of this 'destiny' crap.

I almost wondered if it was SO obvious, that it wouldn't happen XD

Rise to Honor:

Oh my old partner was secretly working and running the triads? I didn't see that at all...not that I expected a good story out of such a linear game.

Seriously, KOTOR II would have been a MUCH better game with the restored content. I think the ending would be a lot better if Atton had been tortured + killed and Bao Dur sacrificing himself.

I agree with quite a few here, namely that Bioshock, System Shock 2 and KOTOR have some InsaneSauce twists in it that completely took me by surprise.

I always thought that the one in FEAR is kinda good...
That you're really the brother of Paxton. i.e. you're Alma's first born son. Well at least you traded in the flesh-eating memory-gaining bit for the rather over-used bullet time.
...as you have to piece it together yourself, with no real 'bam this is the twist' part to it.

The random twist in S.T.A.L.K.E.R. should also probably get a mention, as completing the related quest after the twist seems rather suicidal.

I hated that. It's like Valve decided near release that not enough happened in Episode 2 and threw something in at the end.
two Handlers (that's what they're called right?) just barge in and eat a guy's brain then get scared off by a robot that they could imobilize fairly easily.
They're called Advisors. I mean, the ending scene with the striders and the bombs that takes FOR--EVER was "anticlimactic" or something. I guess they had to do something that would make you want to buy Episode three which they better release soon or I will go fucking insane.

Am I the only one who saw the revan surprise from a mile off? The game couldn't stop yacking on about the force being used to wipe someone's mind and how it changes people, add to that your character has no history but when around bastila she has flash backs of a fight with revan because she is ultra force sensitive.

For a more surprising plot twist there was going to the jedi ruins in kotor 2 after assembling the jedi council, only to have them randomly try to kill you, then kreia pops out of nowhere and kills them.

Oh and Demon_Zeya, never say "hella good" again.

I really like KOTORs twist even though it made me feel stupid because I should have seen it coming.

Final Fantasy Tactics

St. Ajora, the games "messiah", is the main villain.

I get the feeling Square doesn't appreciate Christianity and the Catholic Church.

Bad - MGS2

Hideo Kojima explains, through a rampaging metal gear, that people around the world were tricked into playing the game by promising them Snake, only to deliberately piss them off by making Raiden the main character, thus making this triple layered plot that breaks every fourth wall tendency horrible by essentially displaying one major and bottom line fact... the game no actual plot other than "Oh, we basically copied the same details from the first game and wondered how long it'd take before you noticed"

Edit - The glitching game over screen was cool the first time though. The second time you played through it however...it became rather annoying because you knew you weren't dead.

Good:

Bioshock- It was a great plot twist, now if only the ending didn't suck so hard.

Meh:

Assassin's Creed- You can see the plot "twist" from a mile away but the story is actually good. Yeah I know it's an "ancient cult" but I think assassin's creed pulls it off pretty well and the piece together the story yourself element is a nice change.

...That was the whole point though. Raiden's story WAS a copy of MGS.

Bioshock has an awesome plot twist...one which I will not go into detail on, as i feel even putting it in spoiler tags would spoiler it somewhat
Dude, who hasn't played that yet? And yeah, that one took me totally by surprise too.
...I haven't played it yet

Best one either has to go to Bioshock or Jade Empire.
Jade Empire was too cliche for me.
What was? The entire game or the plot twist? I mean, the twist itself wasn't all that original, but they set it up so excellently. You feel like you're about to finish the game right before it happens.
