Not necessarily the biggest game fail.. but almost everytime I get a green shell in Mario Kart, it doesn't hit its intended target.. only then to bounce off a wall and hit me. GRR

Err.
Well on FFXI, a group of 8 of us where doing this mission (RotZ one where you have to touch all the rock things for anyone who knows). So one of these, the one in... Tarrigan? spawns two skeletons. 5 of the people in our group were afk and the other two were just killing stuff that might aggro us when we spawn the mobs.
Now, I had invisible on which means that basically, mobs that aggro by sight can't kill you and you can't activate things like open doors, check things around you etc. I clicked the thing out of boredom and didn't relise the spell wore off. We died. Everyone was furious with me but I just denied everything.
Later that night, I fell asleep in the middle of a fight. They didn't like that but they were being daft and American. Noone turned up to the time me and my friend has set for it so by this time it was 5am our time.

In Earthbound, I used pray and it made my whole party feel strange. When Ness used the Psi Special that turn he one shotted my party, all in the first turn of a battle. It could only be worse if it were a boss fight XD

I was playing Halo 3 and I had the sword in the Pit. A teammate rounds the corner and startles me, so I stab him in the face. Then I turn around, get startled by a teammate and kill him. This went on until my entire team was trying to kill me.
