I often use either history or fiction for names in gaming: Stuff like Auberon, Ahab, Harper, Churchill, Silas etc.

In a similar vein, I always use the name 'Terra Nova' for a ship or vehicle of any kind. I like that name

KP Blue, KP Shad, KP Shadow, KP Shado for Pokémon and other games, and in main series Final Fantasy games, Gian, GoldSilv or just plain Gold, NMatt, and Aurora or AuroraVirus.

Rikket, Anchupom, Freddie, or "I say!"

Rikket is a poor anagram of my full name, anchupom is just an amalgamation of letters that I have assigned as a name, and Freddie is my actual name. Ok, Frederick is, but that's for dipshits. No-one likes the name Frederick, not even my parents.

"I say!" is just for giggles, as it makes everyone sound posh, if they address you by your character name.

Sean when I'm not feeling clever or when the game is like Mass Effect.
Cadegon or Claymore when I feel like getting a little in character.
Alucard if the character has literally anything to do with vampires.

My wife always chooses Marion (the name of our future daughter) for her first character. My personal choices always vary. I guess I've named two characters Orion.

I use the name Srimose for my elven characters usually. It's original and it sounds cool. :)

Um, I go through phases with names but some I've been known to use in the past:
Asorac
Maves
Dars
Trai

I've recently adopted the name Espen into use. I like this one a lot.

Male ones:
Issac - One of my favorite games from my childhood was Golden Sun.
Ciel - my first DnD character was named Ciel and now it's my go-to name for magic-users.
Aric - Original name, used mainly for rogues.

Female ones:
Belruel - From Penny Arcade, i like the name.
Isabel - I just really like the name Isabel for girls.
Aria - Fem version of Aric.

Either Legoshoes for any arcadey games.
Or i try to grab Achenar for RPGs. The name sounds awesome and i first heard it in Myst.

Axle is always the name of my first male character. The only exception to this is Pokemon where my character is always named Golden because my first Pokemon game was Gold and I named my character Golden.

For females it all depends on how I'm going to play the game/what race they are/how I make them look. ie. I have an assassin in Oblivion I named Stiletto (after the knives).
