I didn't expected to turn this dark honestly :(

Going through the Prison of Hope (3-1) in Demon's Souls for the first time scared the crap out of me, especially the insane prisoners that would attack without warning and hearing the singing that accompanies a mindflayer before you can see where it is.

Other than that, I tend to be frightened of any underwater stage in a 3D game, especially when there's some kind of large creature down there.

The Black House in the second area (after Kingsmouth) in the Secret World. Even without being on the quest and having things happen, that place scared the shit out of me the first time I went in it.

Bioshock's not technically a horror game, right?

That isn't the scariest part of the level or anything, but there aren't any pictures of the ice statues that cast massive, creepy shadows and move when you're not looking.

PS: I know Bioshock has a lot of creepy moments, but I would still argue that the majority of the game is still NOT focused on horror.

EDIT: Oh yeah, and the Dead Reaper level of ME2, when you watch the security recordings of the crew going insane. Particularly the one where the guy thinks he sees some kind of dark, horrible creature living in the walls.

You know the song [http://www.youtube.com/watch?v=NAmlQr79TzU] in God Of War III that plays in Hades? Gives me chills every time.

There are a few pokedex entries that are pretty creepy. There is the one for Yamask.

These Pokémon arose from the spirits of people interred in graves in past ages. Each retains memories of its former life. Each of them carries a mask that used to be its face when it was human. Sometimes they look at it and cry.

Then we got Duskull

Duskull can pass through any wall no matter how thick it may be. Once this Pokémon chooses a target, it will doggedly pursue the intended victim until the break of dawn. If it finds bad children who won't listen to their parents, it will spirit them away--or so it's said. It loves the crying of children.

I found those 2 particularly creepy. Not to mention since it was mentioned to me, I can't stop seeing how similar Clefairy, a normal type, and Gengar, a ghost type, look.

I think it was Black & White 1 where it asked you to type your name in. Sometimes, when I had the camera around my temple, the game would whisper my name. In the same whisper it used to go "Deaaaaath" when vilagers would die.

What. The. Fuck.

When I told people about it everyone thought I was losing my mind.
Huh, I thought it was Black and White 2. But ya whenever a villager was killed the "Deaaaath" whisper was unsettling. That and it never went away, just constantly reminding me of the misdeed whenever I was in the area.

Elder Scrolls Oblivion.

Lucien Lachance.

As stupid as it might sound, the first time he appeared mid-sleep, it jump scared the crap out of me. Did not see that one coming.

Was made all the more terrifying by that damned creepy voice of his.

I was going to post something about Shadow of Chernobyl that wasn't actually meant to happen, then I realised it actually is classified as survival horror.
Well then.
Um.
Oh, right.
The dream sequence in the first Max Payne, the one where you run around on trails of blood suspended in a black void.
And then something that sounds very vaguely like an infant and never should have existed in a universe created by a benevolent deity emits a scream that will make you go pale.
I couldn't sleep that night.

Fudge, forgot about that one.

Puts the shits right up you when it comes out of nowhere, especially when you're not really looking at anything.

The majority of playing through TLoZ: Majora's Mask, I was incredibly confused and creeped out, I was only 8 back then. =V
This one is creepy in the small details. Like how, on the first and second day, if you look at the top of the Clock Tower through the telescope, Skull Kid mocks you in a child-like manner. On the third day, his head is tilted 90 degrees to the right and just ... shakes...
Also when you call all 4 giants, and the Mask sheds the Skull Kid.
It is also a very sad game. If you don't complete some of the objectives for the Couple's Mask. You see either Anju or Kafei alone in the hotel. Hell, even if you do complete all the objectives, Kafei arrives at 4:30 A.M., and you're still aware that they will die.
Plus, no matter what you do, you can't forget that the Deku Butler's son, Darmani, and Mikau are still dead.

The Meat King's Party from Hitman: Contracts freaked my shit out.

Your mission is to rescue a girl who has been kidnapped and assassinate her kidnapper. It takes place during a BDSM fetish party held in an abattoir, which is disturbing enough on its own. Then halfway through the mission, you come across a weird shrine in the basement filled with candles and romantic music playing. Then you open a door and are met with the girl's corpse hanging from the ceiling, blood dripping from every orifice, presumably waiting to be processed into meat. Fucking hell.

Captcha: that's right

Fallout 3, this building

The Dunwich Building

Seriously. This place gives me the chills:

And the basement OH GOD THE BASEMENT
Wow, don't remember this building. But FO3 did have some chilling moments. The vault leaking gas that drove the inhabitants insane, the Garry clone vault, Andale, Point Lookout, Tranquility Lane. Is the Dunwich building really more creepier than those places? Curious and would like to know. :p
Thats definitely one of the creepier bits in Fallout. Like you said, there are alot of things that are off-putting, especially in Point Lookout, (Pint-Sized Slasher comes to mind) but they're almost always in the subtext. Which I think is great, don't get me wrong, but the Dunwich building is the one part I always dread doing.

Not just because it's insanely creepy, but the place actually has teeth.

"WHY DID YOU LET US DIIIIIIIIIIIIIIIIE?"

Man, Psychonauts got dark sometimes.

And yeah, I feel justified including this, because while the game ended up being a terrifying mindmelter, it had all been cute kids and candy up to THIS point.

Also: In The Sims, the ultimate in derpy happy fun times, I accidentally got one of them killed (stupid satellites!). The other Sim then walked around, ignoring my commands, for two days, before walking up to the deaprted's grave, screaming incoherently, and dropping dead.

I... didn't play for a few days.

In mortal kombat 9 when the smily dude jumps out at you from the bottom of the screen at random moments. The first time that happened I was playing it with my mate and we had pause the game after freaking out, and we spent the next half an hour laughing. It was terrifying.

Rayman 2 the cave of bad dreams especially the first part of the boss.
You mean the "get chased down a slope while staring past the creature's teeth" section?

I also put forth "Tomb of the Ancients" from the same game. It's basically a decrepit tomb with the usual haunted-house trappings.

But that soundtrack.

OH GOD THE SOUNDTRACK.
