Personally, I wasn't really "bothered" by A Serbian film, but that's because I view it as "progression." As listed already hear, movies like Tetsuo the Iron man, most anything directed by Miike Takahashi, or most Japanese "shock horror" have been tapping into the "WTF?" category for a long time now.
One that comes up often for me is a Miike film called Visiter Q. That thing... just... what the fuck.
I will say that I'm surprised none of the Hellraisers were mentioned. While seemingly "dated" by today's standards, the concepts and ideas expressed in most of them are frankly "cutting edge." I'd say that Dee Sniders film Strangeland also hits on that mark a little as well. Though, conceptually out done late by Saw, I do think that the moral expression in Strangeland is more reasonable.
The one movie I often find my self suggesting to people who want a "scary" movie is one that very few have ever heard of and that's High Tension aka Switchblade Romance. I loved it all the way through which is extremely difficult to do for me. I find the majority of modern horror to be extremely boring and lame. I guess the idea of reasonably attractive French girls running for their lives was fun for me or something.
Lastly, although outside the lines of most of the others mentioned throughout, I'd completely support Perfume. I actually feel sort of depressed so many people have never heard of it. While the book is extremely unpopular over here, it has been an amazing influence for many artists ranging from Curt Cobain to Rammstein. Admittedly I'm not much of a reader, but the movie was fantastically done.
