Trainspotting is disturbing just because of how realistic it is, although maybe it's not disturbing in the way you mean it.

I'm a little surprised this hasn't been name dropped yet, but Perfect Blue was pretty damn creepy for me. It's basically an anime version of Black Swan but is somehow darker and more insane. Part of that comes from the confusion as to how much is really happening, or is just being imagined. Sure Black Swan did that to an extent, but its way easier to tell in that film when the character's just tripping balls. Apart from a few moments, it's kinda hard to tell in Perfect Blue, though a lot of the stuff that actually happens is still unsettling anyway :/

Really weird ending though, and not in way you'd expect...

Totally black and white, (thanks to almost a year of post-production), no dialogue, or music, just sounds of the wilderness and some of the actions on screen. It's like a surreal nightmare captured on tape.

It would have been better as a shorter film, but if you want something disturbing, check it out.
Not a lot of people seem to know about the film...I'm not sure if that is a good or a bad thing.

Turn the language to French and turn on the subtitles, and it becomes a French Surrealist Art Film. Also, the French Freddy has a hilarious voice, beyond belief.

If you're looking for a good disturbing movie, check out Super. One of the best films I've ever seen, and basically shot on a shoestring budget.

If you're more into pure shock value, I'd say go for the infernal Threesome, i.e. (in ascending order of disturbedness) The Human Centipede, A Serbian Film and The Human Centipede 2. Although I've kind of found it hard to be too disturbed because everything in those movies is just so over-the-top.
Still, I would say that A Serbian Film actually has some appeal beyond the pure shock value. As the director basically said, it's a representation of how, as a Serbian citizen, you're basically getting fucked from the cradle to the grave, and the main character's progression mirrors the role of the average Serb in the Balkan wars. At any rate, the atmosphere it generates through music and visuals alone is extremely impressive.

This whole explenation of the movie is pretty much bullsh*t, and feels like they came up with it, just in case anyone critized it as just torture porn. (I myself hate that phrase, but it really applies here.)
If the struggles of serbians were really the issue, they wanted to capture, they could have had a protagonist, who is say a factory worker, who loses his job, and struggles to provide for his family, so they don't have to live out in the streets. That could have been an interesting drama, that perhaps a lot of people could relate to ... but hey than it wouldn't be known as one of the most disguisting piece of movie making ever ...
The equivalent would be, if someone wanted to show the hunger situation in Ethiopia, and make a film filled with cannibalism, bestiality and murder .

I will add Uwe Boll's movie 'Rampage'. It is actually decently directed which I know is heresy to say in many circles. It is about some college aged guy who makes a suit out of armor and goes on a killing spree in his town. There is social commentary sprinkled throughout about mass media. If you only know him from his cheesy video game movies, check this out.

Also almost anything from Takashi Miike will suffice. 'Visitor Q' made me never want to watch a horror movie again, because how sick it was. "Ichi the Killer' is disturbing, but at times fun because the yakuza guy acts like a crazy, masochistic hair metal guy. Then there is that famous one of his that makes Stephen King's misery look pure and innocent. The name escapes me.

Personally, I wasn't really "bothered" by A Serbian film, but that's because I view it as "progression." As listed already hear, movies like Tetsuo the Iron man, most anything directed by Miike Takahashi, or most Japanese "shock horror" have been tapping into the "WTF?" category for a long time now.

One that comes up often for me is a Miike film called Visiter Q. That thing... just... what the fuck.

I will say that I'm surprised none of the Hellraisers were mentioned. While seemingly "dated" by today's standards, the concepts and ideas expressed in most of them are frankly "cutting edge." I'd say that Dee Sniders film Strangeland also hits on that mark a little as well. Though, conceptually out done late by Saw, I do think that the moral expression in Strangeland is more reasonable.

The one movie I often find my self suggesting to people who want a "scary" movie is one that very few have ever heard of and that's High Tension aka Switchblade Romance. I loved it all the way through which is extremely difficult to do for me. I find the majority of modern horror to be extremely boring and lame. I guess the idea of reasonably attractive French girls running for their lives was fun for me or something.

Lastly, although outside the lines of most of the others mentioned throughout, I'd completely support Perfume. I actually feel sort of depressed so many people have never heard of it. While the book is extremely unpopular over here, it has been an amazing influence for many artists ranging from Curt Cobain to Rammstein. Admittedly I'm not much of a reader, but the movie was fantastically done.

Watch Eraserhead. It's not disturbing really... cept towards the end I mean WTF. But the whole thing is an experience and a weird one at that. I recommend it just for the weirdness alone.

I would recommend the 2008 "Dead Girl". It's about raping a zombielike woman. Really messed up anti-rape movie in disguise. IMDB [http://www.imdb.com/title/tt0896534/]

"The Human Centipede" is a stupid movie. No where near as disturbing as your imagination makes it up to be. You'll be disappointed. Just watch this spoilerful review that riffs on it instead.
