I think I have to go with Kingdom Hearts. Both 1 and 2.
But I'm guessing lots of people are gonna say that, so let's go with

Tekken 5.
One of the ones I remember not having the heart to skip until about a month after I'd been playing it. Still not sure why I sold it.
Also, kitten.

Are we talking intro cinematic or just gameplay introduction?

As far as gameplay introductions go, mine would probably be Bioshock. From fear to despair to hope to admiration to fear again in a few minutes.

Cinematic intro: MGS4. When the love theme came on I was fully captivated.
Gameplay intro: Uncharted 2. Started you right in the middle of the action.

I'm going to have to go with Duke nukem Time to kills intro that was so damn awesome i watched it every time i loaded up that game,

also the intro to lost Odyssey was insane

Mechwarrior 2. It's extremely old, yes, but in its day it was just plain mind-blowing. It still rates as one of my favorites.

for its INSANE SPOILERS:
because the "quote" at the start sets the tone perfectly, and because the last scene with the dark room is downright beautiful.
The conception is cool too, but that comes a little later.

for the dark humor and old-timey charm.

for TDR designs. I would pick 2097 if not for the jarring Red Bull ad at the start.

Timeless.

Great cinematic that really sets the tone for the rest of the game. Samuel Barber's Agnus Dei fits really well.

Dragonball Z Budokai 3's intro is one of my all-time favourites, It's just AWESOME!
The last good Dragonball game too in my opinion.

The intro to Eternal Sonatra. After completing the game it was sheer joy to see the intro over and over again.
