For me, the games I would think as being as close to 10/10 would be Bayonetta, Final Fantasy 8, and (modded) New Vegas.

For recent games, the latest thing I had a 10/10 experience with was DiRT Rally. I simply cannot fault anything about that game, and every little feature and aspect of it's development just blows me away. I got it in Early access and boy was I not disappointed. I watched it evolve into the spectacular piece of driving simulation it is today. There is only one tiny thing, which is I wish you could set your own difficulty in career mode, as you race your pants off to get to the top and then eventually you'll hit a brick wall. This might be ok for some, but due to this aspect, it means *everyone* will hit a brick wall, unless you are master-pro-chief-alien-driver god.
Other than that, I'd give 10/10 for Witcher 3. Rarely have I seen a game do everything so good, yet *still* manage to be greater than the sum of its parts. And special mention goes to the developers too - the stance on DLC and DRM and consumer experience is unparalleled in today's gaming market. The sound, graphics, controls, story, design, character, soul, it all just comes together so well in a world that I truly got lost in every time I sat down to play.
Lastly, this sentiment won't be shared by that many, but I really, really like Rainbow 6: Vegas 2. Replayability, customization, level design, terrorist hunt mode, and the ability to LAN with buddies just made it stand out to me, signifying the end of an era of LAN gaming. Yes, I know technically you can LAN through Steam, but games aren't geared towards that anymore. I love this game, the feel of the guns, the difficulty and strategy in it is the perfect balance between modern and old, meaning that although there's regen health, you can literally die in a fraction of a second due to poor decision-making and risk taking. The reward for playing like a boss with good reflexes and tactics is basically just that you don't die.

Baldur's Gate 2: Shadows of Amn. As far as I'm concerned still the best rpg ever made and easily deserving of a 10/10.
10/10 doesn't need to mean perfection, nothing is perfect, but it can signify a game whose flaws are so few or easily overlooked as to be inconsequential to your overall enjoyment of a product.

When we usually see 10/10, it's because the game has filled in it's purpose and functions very well with little to no bugs. Just a good recommendation to anyone interested.
I have my own way of scoring with a top 10 list though, but it functions differently. Since no game can be ever perfect, I'd give a 9 or 10 to any medium that's done something unbelievably amazing, which no game has done yet to me.
So for the casual 10/10, I would list many many games, even a lot of games people have mentioned here already. On my own list, none.

Off the top of my head, I could probably name about eight.
Mount and Blade: Warband- Easily one of the best games I've ever played and excels at everything. Only fault I can think of is dated graphics.
Warriors Orochi 3: Ultimate- Hands down best Warriors game to date with a huge roster, awesome levels, and a cheesy as hell story.
Persona 4: Golden- One of the best made games ever. Fantastic gameplay, characters, and story.
Digimon Story: Cyber Sleuth- This game barely makes it, due to a horrible story, bad characters, and one of the worst translations ever. But the sheer jaw dropping gameplay is enough to push it up there. One of the best grind fest I've ever played and a one that actually gives you easy means to do it.

Well, the last game I've played that I would considered "tenouttaten" was Pony Island... Back when I was a teenager in High School reviewing games in my spare time, The Beatles: Rock Band was the first and only game I had ever given a 10 to. On reflection...yeah, I honestly still am okay with that.
I find the 10/10 score a bit weird. It doesn't mean it's perfect, yet, I still have a hard time thinking of games I'd give that score to. But, turn it into 5 stars or something...that's a little more manageable to me, even if they technically mean the same thing. Yet, 5 stars vs a 10/10 seem to have different connotations. I'd could probably list games I felt deserved 5 stars, but 10/10 games? That's more tricky for a completely arbitrary reason.
Anyways, let's try, I suppose.
Paper Mario: For a while, this was my favorite game of all time. I think it does have a lot of nostalgic reasoning to it though. When I was younger, I was obsessed with Mario and it's universe. And this game helped me immerse myself in what felt like a living and breathing world of the Mushroom Kingdom. This blew my goddamn mind!
Now, it's a bit harder for me to get into...this game seems to represent the loss of something for me now. I seemed to have completely lost the ability to immerse myself in games anymore. Still, I really can't deny that the game is still charming and well written and it also has a fun combat system. It's something different for Mario and it's something that has been a part of me for a long time.
As you might notice, I have a Paper Mario gif as my avatar. So, I guess you know the story behind that now.
I honestly can't think of any other games. A lot of games are definitely 9/10, but not a lot quite gets there. If it were 5 Stars, I could probably fill 2/3 of this message board, but, obviously, nobody wants me to do that. So, I'll just mention honorable mentions.

10/10 would mean perfect, as in, no flaws, right? In that case, there aren't any. A lot of games are good or even great, but none of them fit the meaning of perfect in my book. But if we're talking about the most entertaining/memorable/overall good games, then that's a different beast. These games are to this day still entertaining, and I come back to them almost every year or so:

Metal Gear Solid 3 is the only one for me.
The perfect storm of game, one that Kojima didn't want to make and one where he went against what he wanted to make turned out to be a masterclass in storytelling, varied gameplay, and world design.
A few 9.5/10's though. Planescape: Torment would've been a 10 for me, if there was no combat. Not because I feel the combat was shit (it was), but because it feels so unnecessary. I almost get the feeling combat was included because Black Isle weren't confident enough to release an RPG with no combat whatsoever.
And here's one from left-field, Tony Hawk's Pro Skater 3. THPS2 is the one that gets all the praise, but this one is where I feel the formula was perfected to create and endlessly replayable, insanely fun arcade skater. The one issue I have with it is the bias towards grinds, which ultimately result in vert skating not being viable for insane high-scores.

And I wish getting all the web of intrigue nodes before the end of the game actually did something. Though to be fair the game had been redone to avoid comparisons with Assassin's Creed because of Mercer's then white hood. But oh well, I liked what I got before [PROTOTYPE2] came along and crushed my dreams.
My third 10/10 is Alan Wake, a game that could have ironed out a lot of weird decisions and gotten a restraining order for overuse of the name "Stephen King" but the game was just so bizarre and entertaining at the same time from both a gameplay and horror perspective that I couldn't help but keep playing. The shooting was unclunky enough that I could genuinely enjoy it whenever the game didn't keep going slow motion every 2 seconds just to go "LOOK LOOK LOOK THERE'S THE DUDE, THE DUDE! THE DUDE WITH THE AXE, WHATS THE DEAL WITH THE DUDE WITH THE AXE? RUN AWAY!". But regardless it was still fun.
My fourth 10/10 would of course be two games, Portal and Portal 2. They were humorous, dark, lighthearted, heavy and all around just a nice change of pace from all the shoot-bang with its own silly hootspa.
And for my fifth and final 10/10 as of yet would be Undertale. Yeah the fanbase can be weird as fuck (and I mean REALLY weird as fuck) but that's a problem for all fanbases at this point. I like the game for the same reasons that the Portal games were so great, because of how different they felt. I understand that some may not feel this way but please understand that I've been drowned in games that are mostly about killing at this point (Hell, in [PROTOTYPE] literally ANY FREAKING BUTTON YOU PRESS COULD KILL SOMEONE) so a game in which I can just go "Bruh, chill" is a welcome addition to my library.
Now to stop ignoring my Physics teacher's lecture and get back to work, have a nice day everyone.

Not sure about numerical values, but I had the most fun with Warhammer Online - and for quite some time. It would easily be 10/10 in "your" system.
It is closely followed by World of Warcraft (Vanilla & BC) and Age of Conan, then say, Warcraft 3 & Starcraft close behind.

Honestly I can't call any of my favourite games 10/10, assuming that means shining perfection, but they come close, for example:
Sonic 3 & Knuckles: Special Stages are kind of annoying, particularly the ones for the super emeralds, and can disorientating, also those things in carnival night zone are not self-explanatory and are inconsistent in behaviour.
Final Fantasy XII: Fucking huge and I feel very compelled to do EVERYTHING every time I play it, thus I have only ever beaten it once, and likely won't beat it again until a port hits PC and I get all nostalgia flushed.
Resident Evil 4: The non-gun-aiming camera is a pain in the ass to control; why the hell are there 2 of it, why does it follow Leon's turning but also have a dedicated "look about a bit" function.
Skyrim: Pretty easy to accidentall certain NPC's of certain quest chains with no intention of doing so. Locked myself out of a daedric artifact by going in some dude's unlocked house during the day and... looking at him funny or something? Oh and general TES buggy shit.

It's hard to justify a 10/10, but there are many games that are pretty much as good as they can be.
Legend of Zelda; A Link-To-The-Past. Back in the day, this was considered by many to be the best game in the world for a reason. Action, brutal dungeons, exploration, world-interaction and secret-finding are all present and done well. It's what Ocarina of Time was based on, which is also 10/10, very well done because of the formula while adding innovative 3D gameplay.
Don't Starve. Stylized survival gameplay with no hand-holding. It's brutal, and the player is tasked with figuring out the game for themselves. This can frustrate people, especially with permadeath elements, but no game is for everyone. There's a very helpful wiki to use though and a wide variety of mods.
Skyrim I would nearly include, but I give it a 9/10 because the bugs were sometimes horrid and there were a lot of consolized elements including the UI being shit. You have to mod it to get a 10/10, and that's a lot of work for people to do, both installing and creating mods.
There are many more "10/10" games I could name, but I feel these needed mentioning.
Perfect god-like games that are near-impossible get 11/10, obviously. I can't think of any.

Don't Starve. Stylized survival gameplay with no hand-holding. It's brutal, and the player is tasked with figuring out the game for themselves. This can frustrate people, especially with permadeath elements, but no game is for everyone. There's a very helpful wiki to use though and a wide variety of mods.
Skyrim I would nearly include, but I give it a 9/10 because the bugs were sometimes horrid and there were a lot of consolized elements including the UI being shit. You have to mod it to get a 10/10, and that's a lot of work for people to do, both installing and creating mods.
There are many more "10/10" games I could name, but I feel these needed mentioning.
Perfect god-like games that are near-impossible get 11/10, obviously. I can't think of any.
