Global Thermonuclear War. A strange game. The only winning move is not to play. How about a nice game of chess?

Dwarf Fortress, get back to me when you beat it.

Tomb Raider: Anniversary.

It makes Super Meat Boy look like a a walk in the park. Seriously TR:A is fucking painfully hard, unforgiving and convoluted.

...What? Did you have a pirated copy that ruined the ledge-jumping or something? I had no issues with it at all... Seriously, I'm very interested in why you found it impossible.

-Subterranean Animism: Often called the hardest in the Touhou series, and for very good reasons (but without bad controls or glitches, just like every other Touhou game.)
-I Wanna Be The Guy: Mostly about learning where the traps are, but surgical precision is also required for several sections (such as the Ghosts 'n' Goblins world.)
-Syobon Action: Like IWBTG, but Japanese, based almost entirely on world 1 of Super Mario Bros, and it stars a kitten (not that that helps when you see you have minus 20 lives.)
-Yoshi's Island DS: The game starts off by giving you a whole lot of 1-ups, but you will need all of them to win without continues (the main game is beatable, and I'd know, but as for the Secret and Extra levels...don't even try them.)
-Superman 64: This game needs no introduction (or extra text in brackets.)

That last one is a little harder to get, but the other four should keep you playing for a while. At least, until you admit defeat and vow to never play them again, but you still have this niggling corner of your mind that wants you to go back and beat those games that bested you so many times in the past, just so you could finally say that you managed to beat them...I'm sorry, where was I?
Really? Subterranean Animism? I found it quite moderate in difficulty.
Also, i see you Guy and raise you I Wanna Be The Boshy

Edit: Never mind. I was responded to with 'I found [Animism] enjoyable', but he changed it before I could say anything. Or maybe I just misread. Whatever the case, this is a moot post.

Edit 2: Mushihime-sama Futari. First two difficulty levels are 'normal-level standard shmup' and 'normal-level bullet hell shooter'. Then you go to Ultra Mode, and the enemies all use patterns to put Flandre Scarlet to abject shame. And it can get even harder with the Xbox 360 version.

SA? Yeah it's definitely hard, but are we talking Normal or Lunatic? Anything below Lunatic isn't unfair hard like IWBTG. I don't think they should be grouped together, honestly. Not a big deal either way of course.

Got my copy off Steam thank you very much.

Two words.

Fuck. Egypt.

Seriously, the Great Pyramid had me set down my controller and just not come back to the game for a week.

Anniversary managed to be even more difficult than the original and going from TR:A to TR:L is like jumping from IWBTG to "You have to burn the rope."

Not to mention those Relics were damn hard to find in places and the added complexity those added to the area design could make some of the levels quite confusing.

I beat Super Meat Boy in three days, 100% Light and Dark. I can speed run Ninja Gaiden. I am not unfamiliar with difficult games or mastering them and TR:A was one of the hardest I have ever played. Great game, fun, beautiful, amazing puzzles and environments. I have played and beaten all 9 Tomb Raider Games and Anniversary is without a shadow of a doubt the most difficult of the bunch for its puzzles and level layout, especially for Time Trials.

These weird titles aside, I found the S.T.A.L.K.E.R games punishingly difficult. I still haven't gotten anywhere significant in any of the ones I own.
S.T.A.L.K.E.R games are only difficult to start with. They actually get easier as they go on due to your equipment making a huge difference.

Do yourself a favor and get the Complete mods for the series. Not only do they fix a ton of bugs and update the graphics, but it also adjusts the difficulty curve making the game easier to get started and more difficult at the end.

Well since it hasn't been posted yet:

Beatmania IIDX. Seriously, there is no harder rhythm game on the market than this one, and I don't care what Guitar Hero fans think. Nageki no Ki. Look it up on Youtube some time.

Sorry to be predictable, but Battletoads on NES. It's an awesome game, but I've never beaten it. I've ALMOST beaten stage 9 without using any warps though, which is much farther than a lot of people get.

Also, Ninja Gaiden III for the NES. Ninja Gaiden 1 really isn't that hard compared to the third one. I can beat NG 1 no problem, but the limited continues you get in the third game, and the absolute cruelty in how far it warps you back in the stage even if you just DIE rather than get a Game Over really make it the hardest of the three.

Also, Prinny 2: Dawn of Operation Panties, Dood! (on normal mode or hard) for PSP. I beat everything in Prinny 1, so I was no fool when it came to the difficulty of the Prinny games, but Prinny 2 was OVERLY cheap with its level design, and some of the enemies made the stages nearly unplayable. Even after using all my tricks I learned in the first game, I still had to lower it to "baby mode" during the last couple of levels because it was just too effing impossible.

Korean MMO called Mabinogi

It was long ago ported to the US, and has been nerfed twice due to how unforgiving it was for new players

Not the game for you unless you like dying... or glitching >.>

other than that, the aforementioned IWTBTG, and Demon Souls was pretty brutal too

Commandos: Behind Enemy Lines. There is exactly one solution to every level, you have to perfectly coordinate each of the up to 6 people under your control, and sometimes winning is a matter of pixel perfect timing.

Yeah, well, what I'm saying is that most people who play it will probably never beat the game on its 'normal' difficulty (Hard, in the case of Guy.) As for 'unfair hard', I think SA passed that point with Mizuhashi. And then kept going after Komeji, particularly with Reimu-B...

Edit: Forgot to put this on my first list: Shoot the Bullet. Another Touhou game, but this time with a gimmick that makes it even harder to win. Couple that with about a hundred completely insane boss battles, and you have yourself a nigh-impossible game.

Edit 2: Super Metroid Impossible. Everything you thought you knew about both Kaizo hacks and Super Metroid won't help you win this time. Spikes are everywhere, doors change colour, upgrades are swapped around, and timers are shortened...have a lot of fun.

Try soloing to 75 as a Monk on FFXI. Those who played before Promathia came out might have an idea of what I'm implying.

Everything mentioned so far pales before the far, far more unforgiving I Wanna Be The Guy. Which has an even harder fangame made from it.

Go ahead. Play it. It's a free download. If you make it to the first boss before giving up, your gaming gonads are bigger than mine.

It's a free download. What do you have to lose besides your keyboard when you HULK SMASH it.
It's really not that hard, It's just about repetition and memorizing all the traps

I beat it on hard =D

Almost any game ever made by Cave.

You think Touhou on Lunatic is hard? ... It still is. But Cave makes games that are way harder.

Let us go with DoDonPachi (any game in that series) as our example. By default, the game is about as difficult as a Touhou game on Lunatic, if not a bit harder, mostly owing to the fact that you get less bombs and lives to work with.

The fun doesn't stop there. Now imagine that, in order to truly 'beat' the game and get the best ending, you have to complete certain absurd requirements. Like, die less than twice. Or don't bomb, ever.

But oh man, THAT isn't enough for the good ending. No, now the game drops you back at stage 1 again and you need to play it all again. On Hard mode. With an extra boss at the end.

Keeping in mind that Normal mode was as hard as Touhou Lunatic, if not harder, and this game's hard mode is exactly twice as hard.

Ick.

Addendum: Referring to post-Touhou 9 Lunatic here, as 10 is when Touhou really picked up its game with Lunatic.
