Making something like a Kill move random doesn't make it awesome, it make it weak, insulting and unfair. No, it introduce a random chance that the winner would just being lucky despite the level of skill of the opponent. It can be implemented, but it should Always come with a price. A good example is the Guilty Gears series. The Insta-kill move are bound by the tension meter and if you didn't land the attack or get knock on your ass, you lose the tension bar. Beside, it's no fun to play a game where the variable of the game are taken from your hand, you never know if you deserved to win or did it by pure luck.The opponent got a lucky punch in Kimbo's face and he slipped. The dude then capitalised on this and kept him down. Adding random events like this into a game make it more realistic.
Answer me theses:I'd be willing to bet it was a design choice. It's not like it happens every other fight. The guy said he had a lot of games that night and that this only happened the once. And hasn't happened since.

How can you make a fair fight when the variable of each hit are random and that any moment, you could pin the opponent by luck?
How can you be sure that it isn't triggered by a player or that the player can get it to work when he want to?
