Usually I use Hook. Sometimes I make an effort and come up with an epic fantasy name,I do this with all my characters on games like Oblivion and Fallout 3.

When I start a JRPG I come up with a theme and all the characters names follow that theme. In other games I'll call them 'David' or 'David The Agile', and if they're female then I call them 'Lily'(no real reason, I just like the name). Sometimes if I've used the name David and I plan on using an evil character, I use something evil sounding that I like the sound of. 'Nox' was my most recent.

Well normally I just give them the sexiest name on Earth Alex. But sometimes I think of an original name.

Gregor Samsa (cookie for the ref) or Goblin-Kaiser.
If I play a serious game, then I use my own name or I translate it into english, like here on the escapist. Depends on the mood.

If you ment tabletop games - it will depend on the setting.

i just think of a name that suits the characters play-style ... if that fails i try to think of the same thing and translate it to Japanese and if it sounds nice i pick that ... if that fails i just call them a variation of phoenix35/352/3522 and so on...o.o

I usually name them 'Cat Craver'. My legitimate Darkrai is called Cat Craver. He was the first.

However some games don't allow for spaces in names, so they get names like 'Rabbit', 'Whale', 'Tortoise', 'Pufferfish', 'Owl' or 'Alfonso'.

The latter is my name for Mordecai.

The rest are WoW Characters.

Actually I'm going to start calling my characters 'Fagballs' and I suggest that everyone else does when they can't think off a name for their characters.

I usually name my character on the spot depending on the story. If it has it's own backstory for your character, like very little exploration, I use the name generator or just come up with my own. If the game is flexible (like DnD or Oblivion, or the Fallout Universe) I make up a backstory that I keep in notebooks in case I write short stories about them. Like in Fallout: Tactics I named my main character Jericho, a survivor from Texas and a former member of the Texas Rangers till Super Mutants destroyed his home and family, and joined the Brotherhood of Steel. Also: Shameless Plug: If ANYONE has a good suggestion on where I can go to propose a Fallout Pen and Paper campaign, or playtest new rules, please let me know. I've looked on these boards and am not sure if I could post it anywhere....

I usually always name them after my name: Julio C.

Or sometimes I try to be as LEAST creative as I can. For example my mage in Oblivion is J 2 Mage and my thief is J 2 Theif.

Usually I can't be arsed to be creative so I just leave them with thier original names. Although in FF3, I named my team Link, Zelda, Zant and Ganon.

Namaka is for fantasy games and such.
Basher is for shooters and RPGs like fallout3
sometimes I name a character "I say" so that NPC's sound like the rooster from looney tunes when they talk too me example:

"Basher would you retrieve that item for me"
"I say, would you retrieve that item for me

All depends on the game. Here, lemme give you a rundown.

In all Fallout games, I name my characters Jack. Jack the Vault Dweller, Jack the Chosen One, and Jack the Lone Wanderer. It became a funny running gag, though lately I've been using Bob, because the nickname of FalloutBob sounds like a guy with a Beowulf complex.

For reasons I cannot explain, I always refer to Bartz in FF5 as Joe. (It might have to do something with a different character of mine.) And I abused the Namingway like hell in FF4.

In Shadow Madness, I renamed the worker robot HARV-5 as REAPER-X. Sounded cooler.

Persona games! In the original Persona, main character was Drake, after one of my coolest hero-characters. Protagonist for P3 was Haruko Kuroda, after a zombie-hunting psychic I made. P4's guy: Evan Ramsey, because the glasses made him look much like him. (SMT: Nocturne, speaking of Atlus games, received the character naming of Kazu "Ricochet" Harada, also because he resembled the guy.)

I think the point of this is that this is very influenced by characters I make up or write about.
