I usually use one of my aliases (or whatever you wanna call it):

C117
Dajiro
Moff Muppet
Mr. Binkyspunk

And possibly some variations of those, like DarkC117, or just Moff.

But most of the time, I just stick with C117.

Normally I take my name, and then genderize it. So say for Fantasy, my name is Michael, it will be Mikalus.

If you're talking about Usernames, I usually go with something like my tag here. For character names, I like things that start with "V". I've used: Vondal, Vaedren, and Violet, but if I'm ever anything WH40K related, I use Xanthis.

I come up with names looking at objects around my room. I named my band on Rock Band 2 Red Watch after looking at one of my blankets and my Rorshach Watchmen poster. I came up with FirstToStrike after looking at one my trophies and my stitches on my finger.

I usually create a name depending on how the character looks, if I'm feeling less creative I just resort to calling them Sam.

Individually tailored names, and often back stories for every character I make.

I make up names, it's one of my favorite parts of RPGs.

I tend to use Xorena and Zaren for female and male characters, if I don't just make up a name on the spot.

Except in the Legend of Zelda games, where I pretty much always use "Link." Although I might start using Lunk now...

Nothing for games in general, but I always name my RTS personas Mattleaxe - a play on my name and my most ever-so-favourite weapon, o' course. :)

Depends on the type of game.

If it's a MMO, where I'll interact with other players, I'll call myself Azraellod.

Otherwise, it depends on how I'm feeling at the time. If I'm feeling creative, it'll be something I make up unique to that character. If I'm not feeling creative, I'll either call them Karonos Kaos (if the character is male), Erzsabel (if the character is female), or I'll just leave the default name.

I have a few names I always use, but it depends on the character/game. Khorgul is for warriors/tanks, Alex Astartes for spellcasters and Duriel Jonquil for everything else. Historical (Total War, Knights of Honour etc.) I use Cazier, the original form of my family name, Chazot.

For things like Fallout, where fantasy names are less appropriate I just use my name as either Alex or Alexander.

I usually try to make the name fit the setting. For medieval fantasy, I pick up a Tolkien book and flip through to a minor character's name. For anything else, I usually just use a friend's name. If all else fails, they get named "Jalhira Nailo," because that was the name of my first Dungeons and Dragons character (back when Druid was a PHB class...).
