Could I not just die of old age and have some good friend accompany me?
Though really, if I had my choice in a realistic execution, it'd have to be by firing squad. Heroic, relatively dignified, and painless (I'll be making sure they enlist some marines.) Anything else is just totally undignified, painful, and disrespectful imo, even when it comes to execution.

Something cool so it could be recorded and added on youtube. Maybe mauling by bear (I think it would either have to be a polar bear or a kodiak bear). Dying from skydiving sans parachute would be a pretty fun way to die.
Edit: Actually, seeing that pic from Blade Runner gave me one which, if it were possible, I think would be my choice, flying into a black hole.

First I will be strapped to a rocket filled with a fuckton of C4 in the middle of a stadium and surrounded by about 250 guys with muskets. After all musketeers have shot me the rocket will take off directly into the sun. (it will be high noon) All while Rocket Man by Elton John plays through hundreds of speakers. After my rocket/bomb collides with the sun, the resulting explosion will mark the beginning of 35 straight days of partying in my memory.

Well going off of your edit, if I choose old age, do I have to live in prison? If not, then I would pick to die by old age and have my best friend at my side the entire time until I die. I love him more than anyone.
If I had to pick something other than old age, I'd probably want something quick and painless. Probably a gunshot to the head. I don't think I would want to watch someone inject me with lethal chemicals knowing that it is going to kill me. I would probably ask my best friend or my father to pull the trigger. Unless they didn't want to deal with the emotional trauma, in which case I'd just have an anonymous person do it.
