Before going on the suicide mission, I went through great expense to upgrade the loyalty of every single squadmember. I had heard that the loyalty of one's squad members could affect the ending, so I played things safe.
As for the Normandy's upgrades, I gave it every single ship upgrade available in the game, including the Dermal Regeneration (Yay, I'm pretty again!), and upgraded tons of stuff on the Normandy using resources gathered. I was going to go for that one upgrade that makes it so Unity fully heals your entire squad, but I was too impatient and went anyway.

Anyhoo, once I headed out on the suicide mission, I was lucky enough to take out the Collector ship without any casualties at all. We crashed into the Collector station with only damage to the Normandy, and set out on the mission.
I had Jacob sent on the hacking sub-mission, and made Zaeed the leader of the second squad. I also ordered Samara to escort Chakwas and friends once we discovered them back to the shuttle.
I then nominated Subject Zero for creating the biotic bubble. Tali and Legion were my two squadmembers.
We made our way through the mission with relative ease; Jacob was killed with a headshot by Harbinger when we got on the elevator, and Zaeed gave his life for the second squad.

Tali, Legion, and I fought the final boss and won without much difficulty, ripping it apart with my trusty Collector Particle Beam, and when the Illusive Man came calling, I made the decision to blow up the Collector Station.
My reasoning behind this is basically that you can't use a fire to put out a fire. All that salvaged Reaper and Collector technology might be useful about the Reapers, but I told the Illusive Man no good could come of it and destroyed the station.
As we got back to the ship, Tali nearly fell off the ship and I almost had a heart attack, but she made it and everybody sailed off into the sunset. I made sure not to completely sever my ties with Cerberus, and ended the game.

Overall, I'm very pleased with how things turned out. Only two people died- Jacob and Zaeed- and I never liked them anyway.
