Five players? Holy crap, dood. I didn't lose a single character. That would have been absolutely unacceptable for me. The minute I saw signs of losing a character I smashed the start button to load a previous save.

I haven't gotten that far yet, but i just have one problem: which part do i "Forget" to upgrade so Miranda dies?
None. Upgrade ALL PARTS completely.

to kill Miranda, don't make her loyal (don't do her loyalty mission) and then take her with you to the final boss fight. After the fight, any non-loyal characters die (I believe - never tried it myself). I believe that is the only guaranteed way to kill Miranda.

Anyone else know of another guaranteed way to kill Miranda?

Note: Miranda will ALWAYS survive the ship deaths because she's on the bridge.
There was this one Let's Play I watched where Miranda randomly died during the hold the line section. They were trying to get her killed though, I honestly don't blame them.

Me? I didn't lose anyone any of the times I played through the game.

In my first play through, I lost Thane and Zaeed. They were both loyal, it was just me making bad choices!

None, because I'm awesome.

I thought Jack was going to, as she was not loyal, she survived so all is well.

I'm about to do a playthrough on intense difficulty with a brand new Shepard...I'm rushing through it though so I can only assume a few of them (possibly even Shepard) may end up perishing.

I've played through a number of times (3 so far working on a fourth), and only in one have I lost anybody. I lost Tali (Although in retrospect I was a total ***** to her through out that play through so it wasn't that surprising) and Mordin (don't leave the guy how told you he doesn't like to "hold the line" in charge of holding the damn line!)

My first playthrough I only lost Miranda. Good riddance, if you ask me, I don't like her very much. Second time I lost Grunt and Mordin. :-( and Third I didn't lose anyone.

I had a playthru where I lost Garrus, Thane, Legion and Tali. The first two on the ship because I neglected an upgrade or two, and Tali for making the wrong choice on the station. I don't remember if it was that playthru or another (i think I've had 3, maaaaaybe 4) where I was too late to save the crew as well because I left it took long to go for them. It was weird post game only having Chakwas and Joker aboard; I really missed Kelli :-(

(I believe you're allowed no more than about 4-5 missions after the crew are kidnapped before it's too late to save them all. It's usually just after finding Legion).

My first playthru though I lost no one though as I did every loyalty mission and ship upgrade. I wonder if the loyalty missions make any difference, or if it's purely down to the three required ship upgrades (shields, armour, cannon) and making the correct choices for who does what on the station.

Has anyone got a save game with NO ONE left alive? I'd love to try a game with a Shepard who lost Wrex and Ash/Kaiden in ME1 and EVERYONE it's possible to lose in ME2. Would also be interesting to play ME3 with a dead Shepard.

PS. You don't need to upgrade the ship completely. The only upgrades that matter are the ones I mentioned above.

I didn't lose anyone. Did you even upgrade your ship?
if you don't get the hull upgrade jack dies

shield upgrade legion dies

gun upgrade thane dies

there are a few ways to kill grunt, and mordin so I would say upgrade the ship, and do those twos loyalty missions.
So thats what happened to those three. I was wondering what determines who lives or dies in the end. I didn't realise ship upgrades could affect the survival of my squad, but I guess it makes sense, given how Jack and Legion died. Thane seemed to die from simple bad luck though, not sure how a gun upgrade could have saved him.

Didn't lose any squad members and Kelly survived, first try, no reloads, no guide on how to do it. Veteran Difficulty if that changes anything.

It was simple really, do all the loyalty missions, buy all the upgrades, actually put some thought into picking fire team leaders and specialists. Ding, perfect ending.

First playthrough, no walkthrough, no guides, no hints, and I saved everyone. No one dies on my watch if I can help it.

On my first run through, my thumb slipped on the controller and I accidentally chose Mordin instead of Tali to open the door, and he ended up dying. I was so angry at this that I restarted my save from a couple hours before starting the mission (I never save,) and went through it as I wanted, with Legion, Samara, Thane, and Jack dying at the end.
On my second run, Jack died on me.

Nope, not a single one, except on my second playthrough where I wanted to see what would happen if everyone died... it was pretty sad, and it really made me wonder what will happen in Mass Effect 3 if Shephard dies

...died during the suicide mission?

im asking this because i just beat the game and lost 5 squad-mates(Legion, Thane, Grunt, Jack, and Mordin) and i was wondering if anyone else had a similar experience.
if you want everyone to live go to gamewinners.com and follow the video there that shows you who to send were and bang achivment

to be fair though my first play through i lost jack, legion and thane. jack cause i missed an upgrade, legion because i thought tali could lead a fire team and thane casue i thought he would be a good harrasser for the distraction team.

My first playthrough I lost Legion and Zaeed. On my second playthrough I lost no one but Miranda which is exactly what I want to take to ME3. I'm making a third playthrough where I'm not going to lose anyone to give Miranda a chance to redeem herself.

Jacob volunteered, he died
Jack and Legion died before we even landed
Grunt died from Seeker Swarms
Miranda and(I think) Mordin died defending.

I only bought the ship guns upgrade and did about 5 Loyalty missions before I got sick of the Loyalty stuff.

I think that leaves Tali, Garrus, Thane, and Samara left in my crew along with the Normandy crew.
