Five players? Holy crap, dood. I didn't lose a single character. That would have been absolutely unacceptable for me. The minute I saw signs of losing a character I smashed the start button to load a previous save.
There was this one Let's Play I watched where Miranda randomly died during the hold the line section. They were trying to get her killed though, I honestly don't blame them.Bara_no_Hime said:None. to kill Miranda, don't make her loyal (don't do her loyalty mission) and then take her with you to the final boss fight. After the fight, any non-loyal characters die (I believe - never tried it myself). I believe that is the only guaranteed way to kill Miranda.
Anyone else know of another guaranteed way to kill Miranda?
Note: Miranda will ALWAYS survive the ship deaths because she's on the bridge.
So thats what happened to those three. I was wondering what determines who lives or dies in the end. I didn't realise ship upgrades could affect the survival of my squad, but I guess it makes sense, given how Jack and Legion died. there are a few ways to kill grunt, and mordin so I would say upgrade the ship, and do those twos loyalty missions.

im asking this because i just beat the game and lost 5 squad-mates(Legion, Thane, Grunt, Jack,and Mordin) and i was wondering if anyone else had a similar experience.
