Yeah, that'll happen. Not nearly as disturbing as the soldier tossing the woman's baby into the burning hut, but pretty damn close...

Takashi Miike: Shocking people the world over with torture porn since 1997! And you thought Hostel was new. I've only read about some of his works, and they're all like a Tarantino movie, with the gore cranked up to 11 (case in point: Ichi the Killer).

EDIT:
Not really a movie, but a scene in the Hellsing manga where evil Nazi Vampires were sent amok in London. Yeah, baby-eating Nazi vampires.

Honestly, most of these I thought were pretty awesome. The only thing I've seen that I've found disturbing was the plane crash in Knowing, because of how realistic it seemed.

I'm with quite a few people here, anything involving burning, acid, drowning or eyes is pretty much unwatchable for me, yet, give me something like Braindead /Deadalive, where there's literally a scene where zombies tear a guy's entire ribcage out of his chest, I'm fine with that.

I think its long drawn out deaths that freak me out, anything that's a quick death I'm fine with.

That big water tank in Untraceable, along with the heat lamps, it was a great idea for a horror movie, and thoroughly believable, it being based on the theory that people on the internet are a shower of selfish evil fuckers, lol.

Also the one in Cube Zero where there's a dehydrated guy who finally finds water... only it wasn't , it had something in it that broke down the skin, and you see it slowly sliding off his face and arms etc....then you see the bottom of his trousers where it's just a river running down them... nasty.

Oh and just because its a personal phobia, I'd have to go with the inwardly moving walls in saw 5 I think, I've seen crushings before , but never so well done, and the details of bones snapping and being forced out thru the skin etc, just makes it tough viewing.

I haven't seen any intense movies like Saw or Hostel so my ruling is Battle Royale. How so? you might ask. A guy died by a girl repeatedly and viciously stabbing him in the balls. As a guy... that just hurts thinking about it.

I always remember the bit in Red Dragon where the guy is killed and you see him on the flaming wheelchair. It wasn't 'shocking' - I was killing myself laughing (I find films like that kinda funny for some reason). My friends can't see how or why, actually.

I really hated the death in... I think it was a Final Destination movie? Anyway, these two girls get locked inside tanning beds, the heat gets turned up, and they catch fire and burn to death. Burning, IMO, just seems like the worst and most painful way to go to me.

The Punisher movie, with Thomas Jane and John Travolta, did that first, except with a large paper-cutting blade (you know, like back in art class) instead of a cleaver.

OT: Probably anything from the Saw movies, Hostel, or Halloween (like the arrow through the neck? Or bashing the sleeping bagged camper against the tree? Yeeeesh.....)
I'm pretty sure the watchmen graphic novel came before that. I've never found any death sickly, some from sin city are pretty gruesome though, like the guy with razor sharp nails, he had his limbs sawn off and then had his dog eat his half his torso then got decapitated, he never winced through the entire thing, freak.
Oh yeah, I know that Watchmen came before The Punisher movie, but Rorschach didn't bury a hatchet in the kidnapper's head in the comic. He handcuffed him to the fireplace and handed him a hacksaw before pouring gasoline everywhere, saying "Not enough time to cut through the cuffs", implying that the kidnapper had to cut through his arm to escape the fire (the original Saw moment). I was trying to say that, to my knowledge, The Punisher did that head-chopping thing first.

Also, I LOVE SIN CITY! I thought that movie was fucking amazing. Like when the cop gets turned into a Pez Dispenser after getting a pistol slide lodged in his frontal lobes. Astounding movie, amazing comic, great story.

Watch Burn After Reading (won't say who dies cause it'll ruin it but you'll know it when you see it) and everything Funny Games. Funny Games is really disturbing. So disturbing that most people who see it get offended. I wouldn't recommend it usually but that entire movie is just sadistic. Viewers beware.

The part in robocop where the mutated guy gets splattered my a car disturbed me when I saw it as a kid. Also in Full metal jacket the part where the marines are at the end of their training; that was disturbing. And of course that part in King Kong mentioned earlier.
