I'd probably say the zombie genocidist achievement in L4D. It's not necessairly hard, just too damn time consuming, I don't really want to sit and play that long.
Genocidest is easy, especially compared to the real hard achievements for L4D. Just play the new maps (death toll and dead air) on versus, and if you really go for it you can get 200 kills per section...which is 1,000 kills each campaign.

Yeah the gravity gun was easy except for the final bit with the fast zombies :/) and I did the one bullet challenge easily enough...it's just a shame I wasn't given the achievement for either of them.

I was really mad when I got "Did you finish like we do?" or something like that -.- on guitar hero 5.

Endless setlist in Rockband. I actually cannot remain playing a game for more than 3 hours without my eyes wanting to leave their sockets.

I end up falling asleep every time and fail a song.

I have to go with the Yahtzee's Duke Nukem Forever achievement of going outside, meeting a nice girl, going around the world in a sailing boat and having three blonde children.

What? Are you saying Jim Hendrix-level dream-trip games don't count?
Yup totally agree on that one

(bioshock) well the brass balls achievement is infuriatingly difficult. Get through the entire game without dieing once your kidding right. I found it hard trying to stay alive on normal difficulty.
the seriously 2.0 achievement is a bugger to complete. 1,000,000 kills is an insanly large amount of kills to get on gears of war 2
Really? I remember getting through Bioshock without dying once on easy difficulty.

Sap Auteur in TF2, successfully sapping 1000 buildings is extremely time consuming; also My Brilliant Career was quite hard to get.

Some of the TF2 ones are pretty hard, including OMGWTFBBQ!? unless you organise a "staged" killing of a friend.

getting Excaliber II in Final Fantasy IX is a pain in the ass

As for actual achievements: Surviving for a week in Dead Rising is also difficult for me. Haven't got that one yet ^^;

Probably the one coming up for Brothers in Arms, play the game on the 17th of September for 50g. I can bet that I miss it. Then there's play for 100 consecutive days. WHAT?!

Prototype's "Pt" achievement where you have to get Platnium on all challenges.
besides the fact most of the challenges are stupid, but I still love the game.

Little Rocket Man from HL2 Episode Two. I hate that achievement. I also hated the one where you have to assassinate that German general with a pistol in World at War. God, how much times I retried that part ... made me pull my hair out.

Battlestations: Midway Medal of Honor, this is incredibly difficult if you are not cheating.

I don't remember achievements specifically, but there are hard ones I could name if I was on my Xbox 360 right now.
Now that I think; The one you get for doing 100 matches in Battlefield 1943. They don't tell you how many you've completed so far. I know I have more than 30, since I have the one for doing 100 matches, but I don't know how many specifically. And, the game is boring now -.-
Also, Mile High Club seems like it'd be rather easily. I've played the level before, and I could probably do it on Veteran. But, from what I remember, that level was easy, and the only hard part was the end where you had to kill the guy with the hostage.
The reason it gets so much harder on veteran is that they bring the time limit from 2 minutes to 1 minute.

To beat it on veteran you really have to go into some otherwordly mental union with the game... it's like learning a ballet, each move has to be be utterly perfect and performed at exactly the right time, with NO margin for error.

That's why it's so satisfying to finally beat :)
