That happened to me, actually. I could hear her crying, aos I was looking for her. Then I stepped on her. I laughed quite a bit.
Anyways, for me the most unfortunate death happened in Far Cry 2. Bear with me, here. I had this one friend in the game that was really freaking awesome. His voice actor sucked, but the personality of the character was really likable. He saved my life a ton of times, and I saved his. One time, I got into some pretty deep trouble, and he rescued me. I got behind some cover, healed myself, and started fighting. The gunfight must've lasted roughly ten minutes. I ran out of ammo for the weapon I had at the time, a G3 that jammed repeatedly. I took out my sidearm and just blasted away, running from cover to cover, keeping close to my buddy. Eventually, the fire subsided, and I had three rounds left in my pistol. I looked around, and there were five empty vehicles. Patrols were frequent in this area, so five vehicles meant that five teams of three stopped to fight me and Ferrenc. As I stood there collecting myself, I started hearing cries for help.
"Don't leave me here... Please!"
I recognized the voice, and ran to where the cries came from. Pained groaning was coming from behind a barricade. I hopped over, and Paul was lying there wounded. I immediately started injecting him with medical syrettes, to try to revive him.
"Hit me again..."
I injected another.
"One ... more."
I injected another. I waited for a bit, and he wasn't getting up. I injected one final syrette, and he passed away. My history with him was 35. 35 instances of either me reviving him, or him reviving me. I stood up, and the game autosaved. I think that's the first time I actually became saddened after an NPC died. I took his Desert Eagle with me, and put it in a weapons crate. It's still in that crate.
