I always call my dude characters ST or Thomsos, most the time Thomsos. And I always name my girl characters Lady.

I usually make up a silly one, like Dickfart Cumstumper. The last name I made though was just "Dora". Dora Shephard. Couldn't be bothered with Mass Effect for some reason. Maybe it was because I couldn' screw around with the last name.

If male, I name him Ty, my nickname, but if it;s a girl, I name her after my favorite fictional character, Alice.

I have a few default names but god forbid I try and name a DnD or MMO character. That could very well take me DAYS. It's horrible.

At any rate, my default names are...

Male - Sol
Female - Apnea

Stupid name - Fancypants

I usually have some clever ingorant(oxy moron?) name that keeps me laughing every time i play that game

I always name my female character after the next girl I see.

My male characters I name after either myself or the next boy I see.

Seldom do I break this pattern.

They don't have to be real, either.

If the game is Japanese, I look up Japanese names with cool meanings; same goes for Western RPGs.

-As for my Mass Effect character, I named my guy "Rule 34" Shepard.
...His parents met on 4Chan... Well, okay, it was really because I thought you could bed with chicken legs (Tali) when I first heard about the game. Then reality set in, and it seemed as though Rule 34 Shepard would never fulfill his destiny...

...Until Mass Effect 2.

--My Persona 4 MC's name was Kei Yuudai (which means blessed/lucky great hero). I usually stick with that name for JRPGs.

I usually have a hard time thinking of names, in Persona 3 and Mass Effect (first playthrough) I just used my real name; but most of the time I name the character after a Dragon Ball character, usually Goku.

I name my characters after what they look like, a Bob looking character is bob, and a Jill looking character is jill. I am pretty random.

Zarthek, Xaro, or a random/pre-generated name. Creativity is one of my weak points.

Okay...
If its a Fallout-esque game, I'm usually something bland like John, Harold, Jenkins, or Kickass McKillenstein (funny stories here).
If its an Oblivion-esque game, I'm usually something like Kinziel, Ziniel, Zeromaeus, or Sir Slashy the Slasher.

In Final Fantasy VII, I once went rhough an entire playthrough with everyone named Sephiroth.
Imagine the dialogue.
"Oh my God, Sephiroth is going to destroy the world. We have to help Sephiroth cast Holy to stop him. Sephiroth, Sephiroth, come with me. Sephiroth, you stay here."
"Materiaaaaa"
"Shut up Sephiroth"

In games like SSX On Tour, where normal names are not needed but non-modern things would sound out of place, Qwerty (male) and Qaz (female).

Usually, however I will leave a preset character name as-is. Notable exceptions were, when playing Final Fantasy Legend III in 6th grade, I named "Heroine" Addict and "Hero" Deabyk (for the very few that don't get it, Heroin addict and He Rode a Bike), and upon playing Breath of Fire II for the first time recently I named Ryu Poop. I honestly cannot recall why, but it seemed very important that I do so at the time. It was 4 in the morning, in my defense.

Otherwise, my characters are just named after me
