I hold open the door if someone is close behind me, or they're carrying something heavy, or if I'm actually walking with them. Even if there is no one behind me, I'll give the door an extra push, so that it stays open a bit longer.

I do to, but living in Finland no one ever say thanks, we're quite repressed in that way. Never show any kind of emotion or something similar...
