If its someone on my team then i'll forgive them once but if it happens again by the same person then i'll boot them.
If its some asshole on the other team who keeps getting me and tea bags me or hits my body repeated once i'm dead then its on.

OMG yes, especially if they are on my team and have pissed me off. In MW I was about to leave a building and my team mate apparently tried to enter it through the same door. I was being nice and backing up to let him through. Then the son of a ***** shot me and over the mic i hear "move fucker". I wasn't even in his way for more than half a second and was obviously moving back to let him through so that just boiled my blood. I spent the rest of that game hunting his ass down. Now normally after one revenge kill I will let it go, but I just wanted that sob dead, and to piss him off more I killed him everytime he got to a nice sniping position and what I assume was about to take a shot. I got to listen to him ***** on the mic so I know I payed him back well enough. 
I don't go far out of my way to do it, but it can be incredibly satisfying.
If I can use cod as an example, me and some friends are playing a sniper only game on rust, just for a laugh. one guy knifes me, which I thought was fine, an accident or an impulse or whatever, as I was right next to him.
But when the golden opportunity comes, line up a nice hard scope, wait for a good 10 seconds before pulling the trigger to make sure he gets it all on killcam, sees just how long he was in my sights for...

Personally, i think it's kinda childish to hold small grudges like that.
I do have a proper nemesis though. He is my equal in game play and style. He hows I'll be there on defence or leading the assault. I know he will be doing the same.
Taking him down usually makes me cheer but i dont go out of my way to target him.

Well... if I am going on a high kill streak and they cheap kill me or noob kill me then yeah I will go after them, it's not too often though.

Do it to teammates too unless it's something like L4d where I'm CONVINCED that if you do revenge killing, you get downgraded and can only play with noob griefers.

Usually I just keep playing the game if I die, and if I happen to get revenge it's not becuase I just spent the past 5 minutes slowly crouch-walking up a flight of stairs to assassinate them.
But if someone takes one of my guns, I might just spend the next 5 minutes slowly crouch-walking up a flight of stairs to assassinate them.

I only revenge kill snipers. I litterally go out of my way to sneak them and humiliate the cowards. /Rage.
But on anything else I'm pretty good spirited about it. I don't mind being killed. But if it's something silly like lag compensation and such it does make my frown.

Yes. Usually snipers or campers get the SalamanderJoe wrath sent upon them. I guess my best example would be Bad Company 2, running three constantly annoying snipers down with a jetski. A JETSKI.
I would revenge kill more often of Medal of Honor but I get killed 80% by a sniper so revenge is pointless.
