I can only think of one right now.
when Dom dies in Gears 3
That's all I've got right now. I can even think of any moments that were the pathetic kind of sad.

Metal Gear Solid: The death of Sniper Wolf. To me she felt like one of the most realistic, sympathetic characters. This was aided by her friendship with Otacon, which changed her from a cold, emotionless terrorist into someone with depth, whose life truly meant something to someone else. Her death, as necessary as it was, mannaged to grab my heart strings.

Shadow of the Colossus. If you've played it, you know what I mean.

Silent Hill 2
I had to strain to see in the video that I was looking at James smothering his wife to death. No...no, James. You wouldn't have done that, that just isn't you. It can't be...

It seems like it says something that there's only one mention of Aerith on the first page.

I want to say I had some heartstrings pulled during the revelation of GLaDOS's back story, but that doesn't really resonate into a singular moment.

The end of Twilight Princess where Midna leaves and destroys the mirror, and when Carlos dies in Saints Row 2

it has to be the ending of crisis core on the psp for me.

you know he's gonna die. from the very beginning you know he's going to die! but you get attached to the characters and you don't want to complete the game cause you know what's gonna happen and you think there must be another ending where he lives!! but alas, was not meant to be.... i was there mortified for the entirety of the credits :'(

End of Arkham City made me cry so that one wins.
Well Portal 2 did as well but that wasn't through sadness.

Homeworld.

Nothing's left. Everything is gone. Kharak is burning.

I know I put it in spoilers, but really, it's worth watching. It's from a 1999 game, so anyone who would like it has already played it. Really, I encourage you to watch it..

I love the MGS games
That was the first thing that came into my mind when I read the title. I still remember feeling sick when I realized I had to take initiative myself. That alone made the scene so powerful.

When Vladimir died in Battlefield 3. The music, the dead silence afterwards, the way Dima looks back at him several times... I could literally feel Dima's grief in my gut.

Also this scene in Mass Effect 2:

Every time I do that mission, I play the message and burst into tears every time.
OH JESUS THIS MISSION

Way to drive the point that our parents can only protect us for so long home, Bioware. When I found Rael's body and Tali was frantically trying to check his vitals, I pretty much lost it.

I cried when Zeke died. He was such a likable guy. Words can't express what i felt.
I never cry because of videogames, but damn was it close at that point.
It was one of the only things I liked about the Evil story.

Anyway, others include
God dammit John, why'd you have to go and get yourself killed? I really thought you were a likeable character. I'm not sure if it's possible, but in the slo-mo before you he gets gunned down I really tried to find and aim at that bowler hat bastard. ;_;

When Kiel dies.
Well, he doesn't die, but still.
It was a touching scene. Oh, and the ending.
