I reckon the crackheads/ teen runaways that crawl in Condemned.
The angry alien fish on Half Life
The weird creatures that drag their claws in Silent Hill. I played the demo, saw one of those coming towards and promptly turned it off. Hells. No.
The allosaurus off of Dino Crisis 2
The t-rex and allosaurus from King Kong, when you are Jack.

The entire cast of Shadowman.

God, that was a good game.
One of my all-time favorites. Not the same on PC without the music though =[

I like Legion, he's pretty scary. And Avery Marx, who I just call Johnny... Those are the only ones I consider the satanic and mysterious type of scary the others are the "I'm big and tough" type of scary. All the deadside enemies are all the evil type :p

Fast Zombies from Half-life 2. I can handle all other zombie type creatures... indeed I dont mind most scary enemies... but fast zombies its partly the noise and partly that one of the traditional Zombie weaknesses (Being slow) has been removed. It's the same reason I find 28 days/weeks so scary.

I also found Morrowinds Corprus creatures really scary... but thats partly because every single NPC I'd talked to about corprus said it was incurable and terminal and having played Morrowind for long enough to realise how harsh it could be to players I fully expected you to be able to catch it and die of it. Once I realised the creatures didn't pass it on (at least, not to my experience) they became alot less frightening.

Just reminded me of Morrowind, when during the main quest you get
chucked to an Ashlander village on the North coast of Vvardenfell, near a Daedric ruin. The ensuing journey up there (at night, in the rain, my first really 'epic' dander about Morrowind too) is still one of the scariest ever. I was underlevelled and relying on my moderately awesome Sword of White Woe to cut through the ash monsters and cliff racers, but the real panuc came from the distant red/blue glow of the atronachs. Given all the fear the game strikes into your heart about the dangers of Daedra worship, can you blame me for turning the game off and not returning to it for weeks>

Wow, there's a couple of them. Probably the top scariest enemy, sadly, were the weird mosquito baby things from Doom 3. Just the little noises they made and the fact that it's this innocent baby with claws and mosquito wings jumping up trying to slash you.

There's also damn near ANY psychopath from Dead Rising, particularly Adam the Clown and the GIGANTIC retarded butcher guy.

And ashamedly, the level 343 Guilty Spark from the original Halo. It's my all time FAVORITE level in ANY FPS, and I still get creeped out by it. Just the fact that pretty much EVERYTHING you pass is already dead, and seeings shapes moving through the mist that you hope are Marines. Then going into the Covenant base and finding piles of dead aliens and hearing strange chattering noises. Then finally you are trapped in a room with swarms of parasitic egg things that explode when they touch you, and lumpy recreations of enemies you'd just gotten familiar with.

While on the Halo vein, I guess just the entire level of The Library on anything harder than "normal." I think most Halo fans will agree with that.

*Also, the entire World War 1 section of The Darkness scared the HELL out of me, but particularly there was a man sitting in a trench who would talk to himself for a few minutes, shoot himself in the head, and then sit back up and do it all over again. And it wasn't so much the character in the game that freaked me out. Just imagining having to be stuck in this trench-filled Purgatory, commiting suicide over and over and over but not being able to die freaked me out to where I had nightmares for the next couple days. Call me what you want, that's f*cking FREAKY.

Geez, well, Big Daddies can be really scary, if you run out of ammo in the open.

Cheiftans from Halo 3 will scare the shit outta you if they jump out from around a corner. I've had that happen. I need to pause the game to recuperate.

If you mean like pure FEAR, as in, just the sight sends shivers down your spine? I haven't played much of those games, but I would definitely agree the Pyramid head is the scariest.

Oh yeah! Chainsaw dude from RE4! Those bastards are tough! Even if you use Action Replay, they can still kill you!

Why aren't I scared by headcrabs of any kind? They all look cute and fuzzy to me. I like to cuddle them and then whack them with the good ol' crowbar to remind 'em who's boss! (the standard zombies are hilarious! YAAABBA! MY ICING! XD)

The Broken Neck Lady In Fatal Frame 2. I couldnt even look at a picture of her....
Oh, god. Yeah. Most of the female ghosts from that game made my skin crawl.
Ever played Fatal Frame I? There's a Blinded Woman who moves wicked fast if she can hear you moving, a little like the blind monks from Fatal Frame II. Freeeeaky stuff.

The Hell Knights in Doom 3. Smashing down reinforced doors, bursting through the floor. And never in the direction you would expect it from. And unlike most other enemies, they DO beat the shit out of you and take large amounts of ammunition to kill. At least till you get the Soul Cube and the game loses all aspect of horror.

The things, in The Thing.
The build-up to when you first actually encounter them is so slow and you know they're coming and yet it still catches you off guard.
I haven't got far into that game as I've only had it for a short while and I find myself completely on edge throughout the time I'm playing it...
They don't even look that scary, just gross, and yet no matter how much you're prepared they still surprise you.

I dont know if anyone remembers it but in DMC 4 the second boss fight where you start off just fighting the 2 girls and after you beat them for a second he just pops out of nowhere. That scared the hell out of me the first time I played it.

My eyes! My eyes! She scared the absolute crap out of me.

My all time most scary enemy would have to go to the Hammer Haunts from Thief, with their clanking chains, weird groans and skull faces. You can never quite tell where they are coming from.
Clank, clank, clank; whew the sound is starting to receed. He seems to have moved around the corner. I know I'll creep around behind him and then hide behind the furniture as he passes again. Clank, clank, clank; WTF it's getting louder! You turn and he's right behind you!!

The Redeads from Zelda OoT. Those things that would freeze you and slowly creep towards you, nd once they reach you they start raping your face? Those are scary.
