I found the Cherubs in Doom 3 to be really disconcerting. For some reason, I find the Manhacks from Half-Life 2 to be frightening as well, something about whirling blades a eye-level I guess.

It took us a while to figure out he was invincible and just there to make the hedge maze a LOT more stressful. That was sort of unexpected, since Castlevania as a series normally isn't about scary or high-tension moments.
