only enemies i can remember that really creeped me out were the zombies in Hyrule townsquare after you've turned into adult link, dont know why, but they really creep me out, the sounds, the way they latch onto you, and the sound they make when they attack, they made me take a brake from the game for a couple of days first time i played through it (i was 9 at the time).
The headcrab zombies from Half-Life 2 deserve another mention. When they are burnt, the moaning just creeps me out.
But as scariest enemy, I'd say that the marsh bloodsucker from STALKER clear sky is my pick.

I was playing an FPS a while ago(cant remember the name of it), and there was one enemy who had a massive sword, and he kept slowly creeping up on me, holding the sword behing his head. It was like something out of a horror movie! I was so terrified of him, I had to get my boyfriend to kill him for me XD
I probably saw him in my nightmares too.
Now I think I understand why there are less female gamers than male gamers. XD

My first encounter with the vortigaunts in half life one.(i was 9,i guess)
so I was blazing on through the game, picking off enemy who could only melee with the pistol.
then, I reach a kitchen(If memory serves) and I see a scientist get zapped by something in a dark room. So I just go out to meet these guys and BOOM, across the room, hit by a bolt, life down, dead. Didn't even see them the first time, just heard their chant.
It took me a whole week to get through to that place, having drunk through god how many sodas from a vending machine to get my health up and more, and end up staying in the dark room for most of the time in fear of those bastards.
However, eventually got the nerve to charge 'em head on and pick 'em off one by one with a shotgun.
Another one? The tentacles from half life. what barney says still haunts me("They hear you..")
Oh, and second to the guy who said chainsaw guy and regenerators from RE4. Those bastards were cheap

I SERIOUSLY hate the black Headcrabs and the Poison Zombies from HL2. Every time I hear those bastards I get an extreme case of paranoia. I hate Ravenholm SOOO much...
The Kanaren from Rayman 3. I'm serios all their talking of eating and torturing rayman realy creeped me out.
