Condemned. I made it through one level, almost shitting my pants for most of it. In other news, the game has a really good bucket noise.

you want a scary game look up "The Path", its on game tap or you can download it.
I haven't been scared that bad by a game since Silent Hill 2.

Don't know about crapping yourself but I can attest that this game is very scary. Does the psychological scare thing well while at the same time hitting your with the quick jumpy scares too. Also the first couple silent hills got the job done right.

It can't really make claim to being a game devoted to scares, but believe me, The Cradle from Thief: Deadly Shadows is a level that contains more fucking scares than most horror games I've ever played. Seriously, your hair shall turn white, and if ye survive, ye shall become a new man, fit and lean.

LittleBigPlanet... Dear Lord...

Fear and Fear 2 are pretty scary... ish... kinda...
Although if your really thinking about pursuing the Fear series for scares, i'd go with the original. Fear 2's more Startle than Scare.

Fatal Frame 1, 2, 3, and Condemned.

This scene is from the Condemned game.

Oh jesus, don't get me started, I couldn't play the game for the rest of the night after that...

Anyway, yeah, condemned or any Silent Hill between 1-4.

Both Condemned games and Silent Hill 2, SH2 used ambient noise so well there was a point where I had to stop playing. 0_0

HL2 had that creepy level when you where running around being chased by head crab people and the fuckers that ran on all fours at crazy speeds. Probably not the scariest by any means but definitely creepy in my book.

If you don't mind the good old shock value in both movies and games, F.E.A.R 2 will make you say "What the Hell (or F***) on a regular basis.
With that being said I will jump 2 meters high in scenes like this (no matter how obvious it is that said scene will happen)
I am a scaredy cat I know, I know..

For PC, I can recommend Alien Vs Predator 2, System Shock 2, STALKER(either of them), Silent Hill 2 or Cryostasis.
The AvP games are fun..especially when you are playing a Marine with the motion detector..

Alright, so a zombie trying to cram itself in with you in a small space is scary the first time, but you're really too close to a walking Millenium Falcon for that game to be genuinely scary.
I don't know, I ended up saying "Holy Shit!" more often than I hit the fire button.

Carnevil was quite scary for me back in the late 90's.
But it was a rail shooter with shotguns so it wasn't that bad I suppose..
Nope! Carnevil is scary!

Penumbra: Overture and Penumbra: Black Plague. Really two parts of one game.

Towards the end, I had to limit my playtime because I was getting too creeped out. I wish more games would do that to me.

The only game which has scared the shit out of me, although not all the time, to the level where I had to wait for a week or two before I dared continue playing it is STALKER.
Seriously, that game will have you so emersed that you'll be jumping at shadows and reaching for your (hopefully) non-existing ak-47 IRL. While many other games seem to like to scare you in the sense of "Boo! a monster!" the (first) thing that scared the hell out of me in STALKER was a mostly empty very dark room.
Sadly they seemed to lose that in the second half of the game.

Siren Blood Curse anyone? It will keep you awake for days.
I'm downloading that right now they have it on sale. I would say if you have a gamecube get Eternal Darkness, or Resident Evil Remake. Fatal Frame is terrifying and Condemned: Criminal Origins is my favorite horror game of all time.
