My suggestion, is Doom 3. Don't let anyone tell you you can get through it run and gun style with loud as fuck music playing to lessen the impact... I did that most of the time through the game and it still generally scared the fuck out of me. Not just startled, actually scared me; few games have managed to do that, in fact, I don't think I've ever played a game that's scared me as much as Doom 3. It's very very immersive, that's probably the reason.

The Suffering. You were constantly hallucinating and hearing voices. The scariest part was the asylum. Mainly because there's a strong possibility you'll run out of batteries for your flashlight. And that really sucks. I never played the sequel so I don't know how that one is.

Even if you shoot the mannequins nothing will happen. They're there to frighten you, not scare you.

are we talking scaring you momentarily, or constant feeling of dread and helplessness? Because i'd say Doom 3 has both. No other game has had me apprehensive about dark, cramped places, in space, with demons. Some games, you might start out feeling a little creeped out, but you get used to it as it progresses, i.e. Resident Evil (is anyone really scared of those games anymore?), but no, not doom 3, it just gets worse. Even if you play on normal difficulty and you can take a beating before you die, you still feel like the next thing to jump out at you is going to one shot you.

Holy mother of shit, The Suffering made me piss myself. God, those needle people fucked my mind UP! I couldnt sleep for three days until I passed out at school.

I still like the Silent Hill series, it isnt scary for me. I could play that all day and night! <3 Clock Tower made me get up and scream when the murder kept chasing me. That sucked, atleast I thought that they did the voice of the girl's scream really good.

Silent Hill 1 through 3 are up there. It's not that great a game but there were some really creepy bits in Echo Night Beyond.

Also, the hotel scene in Vampire the Masquerade: Bloodlines still creeps me out.

The first FEAR is the best, although the two Vivendi Expansion packs were pretty good. Monolith, in my opinion, is moving away from the atmospheric tension that Vivendi was able to weave together so well.

Seriously, that game will have you so emersed that you'll be jumping at shadows and reaching for your (hopefully) non-existing ak-47 IRL. While many other games seem to like to scare you in the sense of "Boo! a monster!" the (first) thing that scared the hell out of me in STALKER was a mostly empty very dark room.
Sadly they seemed to lose that in the second half of the game.
THis man speaks words of wisdom. STALKER is one of the first games that totally creeped me out. X18 is the best executed horror place in a game i've played so far.

Fatal Frame gets my vote, too.

Silent Hill is a close second, but good gosh.. Fatal Frame had such a seemingly lame premise (kill ghosts by taking pictures of them!) that the scare factor takes you completely by surprise..

I knew it was coming and I still nearly fell off my couch. I started playing F.E.A.R and then shut it off and swore I'd never play a horror game again. My overactive imagination keeps me up all night if I even start thinking about scary/creepy anything.
