Playing resident evil 4 as well, in the sewers, novistador right up in my face from nowhere. Good thing i shot it in the face and killed it, my friends still remember that one.
Oh yeah and the regenerators, just hearing their breathing made me scared.

Playing through the F.E.A.R. 2 demo. Got completely freaked out since I've never played a game that gave me the impression of simply screwing me over, giving me only the illusion of control(also, I've got no nerves to speak of...)

The bottom of the well dungeon in The Legend of Zelda: OoT still freaks me out. Especially the boss.

In Bioshock at Fort Frolic where you go to the sign that says "More Items Downstairs!" and you go down there and there's a power for the people station surrounded by like 3 statues that come to life when you walk past them.

My scariest gaming moment would be encountering the first zombie in Resident evil 1. I was 11 back then, every creaking sound in the house at night freaked me out. And to make matters worse, I had 2 older brothers having a alot of fun abusing the situation. I had a zombie phobia for quite some time. I was even afraid to play Diablo 1 because there were zombies in it.

Nevertheless, I still watched my brothers play the game. But only when their werent any zombies.

New silent hill, trying to avoid a dead nurse, i walk to a dark corner, and when i turn my light there is a dead nurse with her knife up about to stab me. F$%#k!!!!!!!!!!!!

Also doom 3 and dead space when things jump out of no-where and attack u, lol.

Perhaps the scariest moment was when I was playing Silent Hill 3 for the first time, the first and only Silent Hill game I've played for any longer than half an hour.

I walked into a room with a bathtub and a wall-covering mirror. I saw that there was nothing in the room, so I tried to leave the way came in, the only exit. The door was jammed. My heart started beating a tad faster.

I saw the corruption growing from the drain in the bathtub, starting to consume everything. I kept trying to open the door, but it wouldn't open. Slowly, the corruption had spread across the entire room, and in the mirror, Heather's body was being consumed by it was well, consumed until the reflection stood still, even when I was not.

I tried the door one last time, and it opened. I found myself back the grated metal corridors and red and black stained walls from which I came and I realized that my heart was beating rapidly and a cold sweat had stricken me.

That scared me immensely. It's amazing what games these days can do to you.

EDIT:
Honorable mention goes to the moment in the same game where you walk into the bathroom of the corrupted mall and knock on the stall door. You knock three times. From inside the stall, two knocks are returned. You knock again, three times. The response is the same. You walk away from the stall, only to see that the door opens and reveals nothing but blood and corruption. That was a bit unsettling, especially considering that I played the game in the dark.

Was it in the garden area? I had the same thing happen there.

The regenerators in RE4 scared the hell out of me, right from the first encounter where I got my throat torn out. I was so attuned to that creepy gasping noise they make that I heard one from the opposite side of that prison hallway that's about 50 or 75 yards long.

The moment is System Shock 2 where you finally arrive on Deck 4 and
find the corpse of the woman who's been guiding you through the game. I actually panicked when I couldn't move from that spot and the walls started sliding away.

S.T.A.L.K.E.R.: the shit-scary atmosphere of the underground levels, creeping through so slowly for fear of what could be behind the next corner. Or any time you get jumped by a blood sucker out of nowhere.

One time I was just leaving a bandit base after a long, drawn-out fight, carrying the spoils of victory and going round all the dead bodies I could find (marked with grey dots on the radar) when I noticed there was dead body in a little 2-storey out-building that I hadn't looted yet. So I go in, climb the stairs, go into the little box room where the radar dot is and... there's a skeleton. Looks like he's been dead for some time. Immediately my brain is shouting "it's a trap!" and not two seconds later I hear that unforgettable noise like no other: the growl of a bloodsucker. "The son-of-a-whore tricked me!" Scared out of my wits and veins full of adrenaline I spin round and pull out my AK just in time to drop the monstrosity, and only lost about half my health.

The first time I saw one in game was in the Wild Territory at night. Just beyond the first building you go through after leaving Bar, I glimpsed a tall, gangly figure walking around. Didn't see much detail, just that it was obviously humanoid, but not human. I stood still and it walked on without seeing me. Maaan.

The second time was in Wild Territory again, this time coming back from Yantar and I'd just cleaned out the 3- or 4-storey half-constructed building where the bandits like to hang out. From the top I could see it wandering around down below. It would uncloak for a couple of seconds, see me, then cloak again and run around the railway buildings for a minute or two. It didn't understand stairs, so I was safe up there and I tried picking it off with my AK but it was hopeless; I knew I would have to go down and face it at ground level. A similar situation happened in Cordon, at the abandoned mill or whatever that building is, just south of the railway, I was on the roof and this bloodsucker was running around down below. They have this knack of always coming at you from behind. So then you spin around, but somehow they know to then come at you from the direction you had been facing originally :)

And underground at Agroprom, there's more of them the second time you're there. I was in a position in a corridor where they could come at me from the front or from behind, and could hear one walking close by, breathing heavily as they do, so I figured the best thing was to wait for it to come to me. So I stand there, every few seconds I turn 180 degrees in case it's coming from the other direction. Then suddenly I turn and it's there. Right there in the corridor. It sees me. It starts running. I wait until I can see the whites of its eyes to unload my pump-action shotgun into it :)

I love when you meet different monsters for the first time, like the Controller or the Poltergeist. And then at the brain scorcher when it really starts fucking with your head :)

Also whenever I see someone wearing a welding mask, on TV or whatever, I get a little spine chill reminding me of Yahtzee's Chzo series.

Definitely agree with SS2. Still remains one of the scariest games I've played. Hell, the cyborg midwives scared the crap out of me too, but not as bad as the rumbler in the indoor pool on the rec deck. Nothing can compare to the scariness of spraying at it madly in desperation then hearing the *click* "Weapon jammed."

Also Dreamfall, tip-toeing around in the Border House, with the girl connected to the dreamer, and the sleeping huge bald man with creepy eyes who attacks you if awoken. All leading up to Room 201, which has been suitably built up with prophetic dream sequences so you are so shit scared in anticipation of what you will find there, in the cupboard.

Yeah that happened to me in oblivion (7 hours of play O.O) and assasins creedc(meh, only about an hour, but still annoying.)

I have a couple of scary videogame moments, like in fable 2 near the end, when your a kid again and your playing with your sister and shit, and then you hear music in the night, and go to investigate, and she keeps telling you not to go, and you go past the fence out of site and she just Gives a blood curdling scream of despair and its raining blood and theres loads of people impaled on spears and lying dead and everything goes red and...blech...I just dont like that bit...im not squeamish, it just...creeps me out. For some reason I find it wierd though,that im not creeped out by the hospital in fear and dentist in bioshock and those things...and its ridiculous to be freaked by fable.

and once in either morrowind or oblivion...probably oblivion, I was on my way to a city during the night for a side quest, just walking, I was on a mountain, fought my way up, and saw it in the distance so I went on fighting a bit down the mountain, just some pixies and imps and a whatnot, and saw a dark-bricked house, decided it would be a good place to sleep until day, (couldnt wait with enemies around '-.-) and loot whatever I found in there of interest, So I walk in and go down some stairs...god it gives me creeps just thinking of it in detail, I saw some bones , then went downstairs and found some charred skeletons, and some half eaten meat, and i found a closed door, and heard growling and screaming from inside. god I ran so fast and turned off the game.

I think the most horrified I've been playing a game was playing Scratches and it's only a T rated game. I had previously played all the Silent Hill games in the dark so I thought I would start to play Scratches in the dark. I was on the second day at night where you're dreaming but don't realize it. I went into the African exhibit room cause you're being drawn to it. I opened the door where the evil mask is and it was GONE. I started to get nervous and my breathing increased. I was like, "I'm getting out of here!" I turn around, open the door, and the mask is right on the other side of the door. I freaked out. I was gasping for air. That's when your dude wakes up and hears noices from the basement and goes to investigate. You climb into the furnace and crawl all the way until you get to a sort of barred up opening but you can just see a faint light and nothing else on the other side. After staring at it for a couple of seconds I tried to turn around and realized I couldn't. I was like, "What the...?" Then all of a sudden person walks right in front of the opening and scared the crap out of me. I was screaming, "OH CRAP! OH CRAP! OH CRAP!...." over and over. I panicked and flipped the lights back on. For a few days, every time I would be about to open a door I would get nervous thinking something would be right behind it.
