Back when I first played Resident evil 2, running down that hallway after the shutters go back up and the arms come out at you, I dropped my controller and bailed out of the room, leaving poor Clair to her fate...
There were some great scares in that game...

a few things i can think of right now, Homeworld: cataclysm, the cutscene where you first witness a ship being taken over by the beast, the screams of horror from the pilot made the hair on my neck stand on end.

Secondly (this is an old one) the 7th guest, there were a few scary moments on that game but some that i remember most are the basement maze, you are wandering through the maze and every time you go down a new hallway stauff, ( the baddie), taunts you and tries to lead you to dead ends. and when you quit the game you hear stauff scream "come back!" and i almost thought he would pop out of the screen.

also in clive barker's undying the game got me so spooked with those little jumper things that every time i heard a shuffle in the shadows my friggin guns started blazing.

F.E.A.R standing in the elevator and alma appeared when the lights flickered, i saw her and the lights went dark, my shotgun unloaded in the dark, and when the lights ame on the wall was swiss cheese and i was ready to call it a night.

S.T.A.L.K.E.R was wandering though some dark tunnel or building (don't remember where) and the game already had me on my toes, pulled my mouse to turn left and accedentaly tapped the "fire" button on the side of my keyboard, gun went off and i nearly shit myself.

Clive barkers jericho: altogeather i thought the game was barely playable , not from being scary but from being crap, but at one point before being able to swap bodies i'm waiting on the team to hack a computer and we're attacked, one of th baddies runs up to me as i'm fighting off a hard and lopps my head clean off, the last thing i see are my own ankles as my head rolls away, creepy.

Fatal Frame. Pretty much the whole game, though the eyeless were the worst.

The straw that broke the camel's back for me though was one random 'ghostly image' spot, or whatever they're called. Took a picture of it and the wall turned into a mouth and screamed at me. Turned it off, never played again.

If i had a penny for everytime i got scared playing Dead Space.

If the hunter (unkillable dude) wasent enough to scare you, they fuck you over one last time in the ending sequence.. I think my heart stopped beating for a second

I could probably name a scare for every RE game:

Zero: First encounter with the leech dude, kind of unnerving but not as scary as some other moments

REmake: Walking past a dead zombie only to have it rise up and attack you as a Crimson, or Lisa Trevor...or maybe "Itchy...Tasty"

2: Seeing the Licker crawl past the window

3: Nemesis breaking through the window

Code Veronica: Alfred the Crossdressing Freak

4: Regenerators

5:
Trying to keep quiet when you walk past the Lickers in the cages. And later on when you walk through a corridor infested with them and presumably its the same strategy

I don't really play survival horrors, but this was kinda creepy

http://www.youtube.com/watch?v=oh_i3musvxQ

from about 0:55 to 1:05 when the body rolls down the stairs...

I just sat there and screamed "WHAT THE FUCK?!"

and it was in the middle of the night....

Yup, happens way too often.
Recently, playing Chronicles of Riddick. I realize it's not a horror game per se, but I was sneaking around, thinking I was being all quiet and stealthy and somewhat in the dark, when all of a sudden gunfire just erupts all around me. Scared the crap out of me.

They entire level. That entire god-damned level was terrifying.
This man speaks the truth.

Also much of system shock 2 is well scary. When you are in the body of the many and just the sights, the sounds everything

Mine would be in fear 1.
I was near the end (proably near the last 10 minutes of gameplay)My guy freaked out and i found myself inside the hospitol i was born in. This freaked me out a little bit due to all the moments with the Ghosts. I walked through the first door and Alma appeared. I unloaded into her with my gun and she dissapered in a cloud of what i belive is blood. A little further in i see Alma again. I repeated what i did and got the same results. Then i got to the last part and saw a locked door. I looked inside and saw alma kill the man inside. Confused over what i was suppose to do i just stood there and tried to open the door. I looked behind me and there i saw Alma face to face. this was late at night and i nearly jumped out of my seat.
Ah yes, a scary moment indeed, surpassed only by the infamous Ladder Scare. If you don't know what I'm referring to, there's no way you actually played F.E.A.R.
dude, the ladder scare made me put the game away for a couple of weeks.

Most parts in FEAR scared the shit out of me, its one scary game.

Another part in a game that scared me was one of the first parts in Dead Space, when I walked into a hallway and all the lights went out, and I could hear something crawling..

And in Resistance 2, when you first encounter the Chameleons (the enamies that turn invisable) and have to walk through a forest infested with them. Freaky as hell, never know when one will pop out and kill you

The worst time i have ever been scared by a game has to be on Resident evil 5 me and my friend were playing through it our first time on co op when we got to the part when you first encounter the reapers. We saw the pods on the ceiling and it got me a little on edge when we walked into the next room we heard one hatch and my friend told me to just run up ahead and he'll shoot what ever comes out of the other pods. well the other pods didnt hatch but a reaper certianly dropped from the ceiling directly in front of me causing me to scream like a girl at the top of my lungs for a good 5 minutes at 3 in the morning.

This.

Also, the ladder from FEAR. You know the one.

Afraid of Monsters mod for HL1. The entire thing.

Backpeddling through an entire level in AVP2 with a friggin army of aliens chasing me.

Half-Life 1: Tie between running from a Gargantua and entering the water with an Ichthyosaur (sp?)

If my mind were working better I'd probably think of something better, but at the moment I'll just say Ravenholm - or at least certain parts of it anyway.
