Hespith's rhyme from Dragon Age: Origins, which you hear over time while you're exploring the depths of an abandoned Dwarven city, which has now been overrun by Darkspawn. Seriously creepy.

"First day, they come and catch everyone.
Second day, they beat us and eat some for meat.
Third day, the men are all gnawed on again.
Fourth day, we wait and fear for our fate.
Fifth day, they return and it's another girl's turn.
Sixth day, her screams we hear in our dreams.
Seventh day, she grew as in her mouth they spew.
Eighth day, we hate it as she is violated.
Ninth day, she grins and devours her kin.
Now she does feast, as she's become the beast."
You'll just LOVE the Gravemind then....

Anyways:

All of Majora's Mask.

I've talked about this so many ties I actually have a template in bbcode that links to some of my disscusions about it for this site, I can't find it right now though, so if you want to read them, just look at y post history on threads about scary games or majora's mask,

This. Shalebridge Cradle might just be the single most scary thing I've ever experienced in any game. But the developers also admited to trying to make it the scariest possible level they could.
I gotta agree here, and now I need to get a copy of Deadly Shadows, is the PC version any good?
I've only played the PC version, but I found it to be well-optimized and controlled back at its' release. Haven't played it since though.

Well, it's $5 on Steam right now, so even if it doesn't hold up well I could do a lot worse for a lot more money.

The Forest Temple in OoT was sufficiently terrifying the first time I played through it.

It's dark in your room. You're casually strolling around a castle. No-one's in sight...When out of nowhere in a booming voice "STOP RIGHT THERE CRIMINAL SCUM" and suddenly you need a clean pair of trousers

COD4: During the single player campaign, when you're climbing the mountain-side, and you suddenly fall but your teammate catches you? Have you ever passed diet coke through your nose before? It's painful as hell.

I've never been entirely scared, but the Overlord DLC for Mass Effect 2 was very creepy overall.

cooking mama, when you mess up and she becomes the devil, seriously I chucked holy water at my sisters wii when I saw that!

I think I saw that in the Official Nintendo Magazine. They rated it as the third scariest moment in gaming or something like that. They counted horror games though...

Ravenholm in Half-Life II comes to mind. It wasn't that scary, but I always dread that part of the game.
Am I the only one in the world who loves that part? I find it hilarious and I often play Half-Life just for that part. I love using a saw blade and my trusted gravity gun to get through it.

Scariest part I can think of F-Zero GX, first mission on very hard and you see you might make it. I always fail in the end though...

The Dunwich building in Fallout 3 scared this shit out of me the first time I went in there. I played the entire game on Very Hard with mods to make it even harder, so the entire time I'm pretty much sneaking around hoping I don't run into anything.

I was in that room with the last audio tape. Headphones on really loud to hear footsteps and I listen to the audio. Right when it's over I hear breathing behind me.

O.O I turn around and there is a ghoul facing the wall just standing there.
Cement Blocks were shat.

Twice in Fallout 3.
First time is when I entered Dunwhich building for the first time and it was occupied by tons of ghouls and hat weird ghost guy. I was about level 8 when I got there. About level 8.01 when I ran out.
The next time was when I was in Old Onley. I picked up a Chinese radio transmission and spent half an hour trying to find the source. After giving up I turned around and started to head back home (Tenpenny Tower, sorry Megaton) when I found out there was a Deathclaw staring me in the face. I had never seen one up to that point so I was pretty frightened. It was the middle of the night both in-game and in real life.

Aside from the obvious mention of Shalebridge Cradle, one other time I SEVERELY freaked out during Deadly Shadows was at these words:

CRUSH AND KILL AND CRUSH AND KILL CANNOT HIDE CANNOT HIDE (and so on)

...First and foremost because those guys really seemed invincible at first impression, and there were so many between me and the exit... O_O

Another scary moment is when the Enforcers come after you. At least until you see them in action... XD

I was also always too terrified to properly explore caverns and such in Might and Magic VIII.

And also Hexen: Beyond Heretic. Great atmosphere, really scary.

I was running this thread through my mind-grapes on the way to work and have some more thought-wine:

Max Payne 1 with the creepy baby stuff.

Fallout 3 had some creepy moments see Dunwich building.

Admit it the bit with Psycho Mantis was freaky as all fuck.

Hitman Contracts had some really messed up moments, most of which were on the first level.

this is fun....

The haunted beach hotel in Vampire: The Masquerade - Bloodlines. Had a few great moments and managed to keep up a feeling of dread throughout the entirety of the quest. It really messes with your esteem because you've been kicking ass as this unstoppable Vampire for the last few missions, and now you're being tormented by a wraith and your only course of action is to soak the damage or evade.
I just played that level for the first time today.

Creepy as fuck!

That should be a pretty good guess, as to what I'm talking about.

most of the times when I have to enter an insane asylum, the one that's still clearly on my mind is the one from Bully.
