I still mourn the loss of Tali.

But as I said in a previous post, I'll keep it like this. I like the idea of having to live with the consequences instead of redoing it to get the result I want.

I'm perfectionist too and did everything. This didn't prevent Tali from dying because there are some rules at the end which are hard to infer without reading guides or FAQs. I chose the especialists well, did everything right (as far as I could tell) and she still died.

But if you're careful you'll lose at most one team member, most probably Mordin or Tali, even without reading any guides. Or you may luck out and have everyone live.

Half of my crew died because i was dicking around and doing side quests after they got kidnapped, and the second time Thane died, he couldn't handle the door. ;/

Oh, yeah, that's true. I did kind of wing it on that one (I just chose who I thought would be best for the job), so I can see the degree of luck there.

Everyone survived for me.
I have heard of an awesome lone-shep for ME3 who has killed almost all his team, and everyone he has met. It's going to be him and Joker vs the universe. What an awesome project! :D

Everyone lived. The character choices are stupidly easy, so unless you forget to get all Normandy upgrades and/or skip the loyalty missions, you must be pretty bad to lose anyone...
In my game, Miranda was still pissed at me, so maybe I just got lucky, but since I knew about it, I didn't give her any responsibilities to make sure she didn't cock them up.

Everyone survived, because I checked all my decisions with a guide. Turns out that I was right at the beginning, but I wanted to be reassured.

Everyone survived but I used a guide because I didn't want anyone to die. I don't use the guide anymore since I now know what to do.

Even Jack who was Unloyal survived. o_o

My first run, Miranda got sniped and everyone else survived.

Then I reset, did her loyalty quest, and did it again with everyone alive.

I don't get why everyone is so obsessed with having everyone survive though. I've had one playthrough and left it at that - having perfect playthroughs destroys the point of the games. I don't want to know the opposite outcome of every choice.
yes they can and it is possible to lose your whole team including joker on a single play through so for some people mass effect 3 is going to be a pretty lonely galaxy to explore.

Surely you'd have basically ONLY play the core missions to do that though? And even then I imagine it'd be difficult.

I replayed the game 3 or 4 times, forgot the total.

After doing all the side missions and ensuring I have 600 of both Paragon and Renegade points so as not to lose loyalty during the two endgame arguments (Jack/Miranda and Legion/Tali.)

Got my perfect ending savegame. No deaths, full loyalty, all side quests.

I'm really interested in seeing what Bioware will do for Mass Effect 3 in terms of character imports and whatnot. Considering Dragon Age 2 was just a broke ass ME2 ripoff, it's a safe bet that ME3 will be the milestone for how Bioware handles the story arcs for their future series. If they mishandle it in any way I'm hanging up my fanboy hoodie.

my first time thru I lost Tali and Jack

second time I lost just Jack

I was glad I was able to save Tali, hate to say it but I never really cared for Jack.

Updated everything, everyone was loyal, lost Jacob. But I didn't care since his writing was terrible and not to mention he was annoying. I was happy everyone else survived though, also Miranda got shot but didn't die, hurray!
