A grenade launcher that fires javelins. Why hasn't it been made yet?
Painkiller had something akin to that.

a gun that fires playing cards, that are razor sharp, that split into more playing cards, that explode after entering the wall/flesh/dog in misfire.

I've been thinking over this kind of thing for awhile...It's hard to find original ideas. But, how about a gun that shoots a high pressure "beam" of water like a water cutter?
Kinda neat idea for showing off those spiffy water physics! And the laser sight which shows where a ricochet would land would be pretty sweet...

A gun that shoots out little fluffy cats.
You need to play that one Oddworld.

Every game needs to have that one from Painkiller that shoots ninja stars and lightning.

A crossbow which fires croquet mallets and thermos flasks full of tea.

A gun firing airbursting incendiary rounds.

The typewriter launcher. Simply for the noise it would make when it hit someone. WHAM *ding*.

A whaling rifle/harpoon gun.

A chainsaw misfire where one of the fuel lines breaks and you have a blazing chainsaw for a few wonderful moments before it sets fire to you hands and clothes.

A shoulder-mounted catapault. Like a stone-aged rocket launcher.

The Power of Love as a means to quell conflicts, to shape growing minds (and bodies) in the player character's image and to guide the thought processes of others without the need for violent coersion. If one could stop a rampaging fiend in a game with a hug rather than a rocket launcher, that would make players feel a kind of power that most of them can never have in reality no matter how tough they are. Breaking Hearts>Breaking Boards and Bricks.
That or Biological Weapons. It's how the West was won!

I thought of a good one (though please let me know if its been done, probably has)

A grenade that allows you to trade area damage for intensity by means of a sliding scale.
By way of example, at one end of the spectrum it could have a tiny blast radius, say 1 metre, but be able to bust through walls. Or at the other end expand the blast area to 10m, but it does relatively little damage, say just to unarmoured targets.

By holding down a button the player would decide how wide the radius is, and the according power.

A Michael Bay gun- a gun that fires explosions, with explosions coming out of the explosions, with exploding tanks coming out of those explosions, etc...

A device that creates tiny black halls in floors or walls that suck in all enemies and then after some time spit them out again in a deformed, torn apart mess.
