Red Dead Redemption and Sonic 3 come to mind. Psychonauts is probably a 9.5. Team Fortress 2 on 360 would definitely be this if it weren't for the bugs and exploits. Minecraft looks like it's turning out to be one. Portal 2 will probably take the cake though.

Games with NO faults don't exist.

That said, my top picks would be: Twilight Princess, Red Dead Redemption, Portal, TF2, Morrowind, AC2, Bayonetta, ME, and Minecraft. Each one has faults, but the games are so engrossing that one rarely, if ever, notices them.

Edit: OH GOD BRAID. HOW COULD I FORGET BRAID.

Short answer: there is no such thing as a 10/10 game. There are flaws with everything...damn quantum fluctuations resulting in an skewed matter to antimatter ratio, which eventually resulted in our lovely blue planet. Wait, nevermind...
Even if I were to arbitrarily assign a 10/10 to my all time favorite game (Silent Hill 2), that would still entail that my second favorite game (Deus Ex) is just a tiny bit inferior, in lieu of being flawed in different ways. Of course, my tastes will eventually change, and there may be some other game I will prefer to Silent Hill 2. Given that I like it more, it would take the 10/10 spot, and SH2 would be reduced to a shameful 9.999999/10. The only sensible thing here is to exclude ridiculous ratings like 10/10, or even 9.7/10! No game is perfect, and pretending otherwise will only lead to mountains of crappy knock-offs.

Shadow of the Colossus, Red Dead Redemption, Tales of Vesperia, Okami, Metal Gear Solid, and Torchlight. I'm not saying these are the absolute pinnacle of video games, just that they were games I enjoyed thoroughly without any of the faults getting in the way. (Arguably, Torchlight's lack of multiplayer is kind of a fault, but that didn't stop me from loving the shit out of it. I play Diablo II single-player anyway, the online community is like 99% assholes)

The list goes on. Then again, those are really just my favorite games, But they're favorites for a reason.

Kingdom Hearts 2. I can't even describe it, in my opinion the only "MUST PLAY".
Eternal Darkness - Sanity's Requiem. Does anyone remember this one? slightly buggy Gamecube-game from 2002? Well I do, still play it yearly.
Nights Into Dreams. Another game I can't seem to put behind me, every year around christmas I make sure to log a full run.

And that's it.. I think...

Bussinrounds is right. A game can have a 10/10 even if it has flaws, as long as its other features make up for them.
Maybe in the world of online reviews, but a true 10/10 should have no flaws. Therefore it should be very rare.

What you descrived is a clear 9/10 (i.e. GTA 4 etc).

Sorry, but no point in having a perfect score thread if we're not going to talk about perfect scores!

Mass Effect 2, Minecraft, and Audiosurf. Honestly, I can't stop putting hours after hours into those three. Mass Effect 2 because I just love the gameplay, Minecraft because I can build ANYTHING, and Audiosurf because I love music. So much.

Portal is the only perfect game ever made. Fact of the day.

Knights of the Old Republic, Half Life 1 and 2, System Shock 2, Company of Heroes, Rise of Nations, Rome Total War... they all come pretty close... but have little niggles that keep them from 10/10.

Well I do not think a 10/10 can indicate a perfect game. The definition of that number should be changed to nearly flawless because without that modified definition no game could ever reach this point. That being said, I would have to say my favorite games of all time, which are 10/10 in my opinion are Jak and Daxter: The Precursor Legacy, Ratchet and Clank, and Portal. These games were great in their simplicity, but also managed to be very deep at the same time. This is of course my opinion.

Guys, if you wouldn't rate anything as a 10 out of 10 on a 10 point scale, then it's not a valid scale. There should be several examples of each point on the scale.

Let's try some 10s that no one else has mentioned...

Legend of Zelda: Link to the Past is my 10/10 Zelda. (seen someone mention this now)
Mega Man X5 is the 10/10 game from the Mega Man series to me.
Dawn of War II (including expansions or not) is such a fun and novel departure from standard RTSes that I'd rate it 10/10. Helps that I love 40k universe.
A lot of people mention Metal Gear, but no one so far has mentioned the portable entries, of which I felt Peace Walker was a 10/10.
Since SSB has been said, I'll say that I found Final Fantasy Dissidia incredibly satisfying and fun, and Duodecium added several improvements, enough to warrant a 10/10 IMHO.
Strange Journey comes close, but The World Ends With You is my 10/10 DS game.

I love the first 3 Kingdom Hearts, but they just miss out on 10/10 scores because of a few problems.

There are plenty more games I'd rate 10/10 but not typing out right now.

(Portal is not a substantial enough experience to rate 10/10)

Ocarina of Time, the second Paper Mario, and both Banjo games.

And there's Metroid Prime. And Portal.

I honestly can't think of anything seriously wrong with those games. Even nitpicking is hard.
