Running away from a mutant Rancor whilst fighting Sith Cultists in Jedi Academy was an adrenaline rush, considering the thing was massive, moved fast and could eat you whole.

zone of the Enders 2, taking down the entire battleship armada by flying behind then and charging up the vector cannon, was adrenaline fueled and I loved every moment, made even better by the music being a re-arrangement of the main theme <3
MATE!!! props for being the only person (other than me of course) to mention ZoE2, every boss battle in that game was full-on. although ide have to say that
the level where you and that army of earth force suits have a massive ruck with bahram to get into the fortress, literally thousands of units, epic EPIC battle

and that is only topped by:

The final final final battle with the FINAL form of Anubis on hard difficulty, that was an epileptic fit of buttonmashing ultra high speed heart attack inducing action, me and a load of my friends spent 6 hours playing that one battle again and again. it was almost annoying because we would always get him a few hits from death and then balls it up and get killed at the last minute, but it was so much fun we didnt even care because it meant we got to do it again XD

that and getting all the hossies in Counterstrike are the most intense moments i have experienced in gaming.

The ending of CoD4.
A number of people have listed various set pieces from COD4 but for me, it was a the 'standard' combat within the US marine levels I found particularly intense, on the harder levels you find yourself pushing up to the next piece of cover, then waiting for the AI marines to follow and take some of the pressure off.

It probably helped that my widescreen monitor covered my peripheral vision adding to the immersion.

The next two are scripted, those scenes in both Bioshock and KOTOR where the twist is introduced midway through the game.

The game can be found here. The fact that it's hacked doesn't change the game much, it just makes things a tiny bit easier. If anyone finds it on a website that isn't this one I would love to see it. Also, I kinda ignored the money by buying a bunch of useless crap and beaming it out of my ship.

Brookhaven Hospital Basement Hallway-Silent Hill 2

I'm trying not to give anything away, but it basically starts out as a simple exit that turns into a mad dash that (upon replaying the game) I discovered took a total of around 28 seconds. But goddamn if it didn't feel more like 28 minutes

It was an otherwise underwhelming game, but the highway battles in Need For Speed: Undercover were some of the most intense, white-knuckle racing I've ever done. 300km/h, through heavy, sometimes oncoming traffic, and forced to use bumper cam. Such fun.

The first town square part from resident evil 4. Shit, that may be my favorite sequence in any game.
