Pretty self-explanitory. Any type of flagrant error worse than a mere RROD or BSOD? Perhaps involving incineration?
Luckily, the worst I've experienced was an in-warranty Red Ring of Death, plus some unplayably-scratched disks that lost me hours of unsaved progress here and there.

This (search-bar approved) topic obviously applies to PCs, consoles, and handhelds of all types.

Well, it was intentional, but my friend took the heat sink off the processor of his old computer. It ran okay for a few minutes, then started to smolder and crashed horribly.

Wow, my 360 did just that. Except that the scratch was hairline and perfectly circular. Made it next to impossible to even detect the scratch until the inevitable fatal error. Goodbye Fable 2, Mass Effect, Halo 3, Armored Core 4, and Enchanted Arms. I would really like those games back sometime... except Enchanted Arms, which is more fun to use as a dog frisbee than actually play.

Blargh my first PS2 did the same thing. I lost about 6 games before I realized what was going on, the worst part was explaining to my friend how God Of War got ruined. I manned up and bought him a new one but it still sucked

I've been given (to fix) a computer that had been hit by a mental power surge before. The PSU, motherboard, HD, RAM and processor were all fried. I managed to salvage the network card and the old MX420 graphics card, but that was about it.

I didn't get it working per se, but I did replace the whole thing!

My laptop once decided to corrupt its master boot record, thus completely bricking it. Those three hours it took to fix it sure were a lot of fun...

I tried to install FaunOS once. I restarted the computer and it came up with an error.
"Missing hal.dll"

I restarted without the drive in (normally).
"Missing hal.dll"

I waited a month for the computer to be serviced and Windows to be reinstalled.

I had the classic disc read error on my first PS2, but that seems common. More recently, my launch 20GB PS3 decided to die in quite a unique way. The large fan that blows air at the top of the unit just...stopped. I had no way of fixing it, and the system could not play select games for more than 15 minutes without shutting itself off. I was able to play everything but Demon's Souls, Sonic Unleashed, and I think Bioshock without it happening, but I had a feeling it was going to only get worse. It eventually got to the point where I could only play PSX/PS2 games without an almost instant overheat, so I traded the bricked system in and got a new 80GB which I upgraded to 160GB.

Unfortunately, the HDD I used to back up all of my saves decided to spontaneously not work, so I lost all of my progress, including at or near 100% files for Heavenly Sword, Sonic Unleashed, Bioshock, Mirror's Edge, Demon's Souls, Resistance 1 & 2, Dynasty Warriors 6, Ninja Gaiden Sigma, Metal Gear Solid 4, Resident Evil 5, Assassin's Creed, and countless other games. Still in the process of getting back to where I used to be in old games, since I refuse to use someone else's save. It sucks, but whatever. At least I get to play the games all over again, right?

EDIT: Oh! On the topic of PC errors, my laptop decided to adopt a "bad image error" whenever I open a program, but this happened after getting rid of a crapton of viruses that took days to destroy. Still have no idea where they came from, but I've left the bad image error since I don't want to have to reinstall Windows or whatever to fix it. The bad image error has been there so long, it now feels weird using another computer and being able to open programs without having to close out the bad image window. I tend to automatically click in the center of the screen to kill the popup even when it isn't there. My laptop also overheats at random, and decides to not be able to play videos every now and then. Fun fun fun.

Does a large and heavy bookshelf falling on my laptop count as a system failure?

Because it was super extra crushed.... and obviously didn't work...

Let's see...

I had a PS1 that decided to start scratching the shit out of my games for no reason one day,
A PS2 that destroyed PS1 memory cards and scratched PS1 games,
A friend who put a glass of milk on top of my PC's tower and then knocked it over, destroying it, and
An NES that got peed on by my little brother when he was 2 years old

Not the worst of luck, but pretty bad for most people's standards.

My video card decided it no longer wanted to display an image, after replacing it I took it apart and it had a number of burn marks on parts touching the metal.
Not too big a problem but two months later my monitor randomly decided to refuse to display an image.

Slightly related, my computer has two USB ports at the front of the tower that for some reason destroy the device plugged in.

Well... I booted a pc with the harddrive open once... Worked for a while, dust started to fall in and scratch it but it still worked. Though when I touched it with a permanent marker it instantly crashed.

Halfway through finishing a level in GRAW2 for the ps3, my system froze and I had to restart. Works fine though.
I've been very lucky, I have to say (and this is the new 120 slim one, btw).
