Thanks to good ol' Doom 3, I'm terrified of in game spiders. Real life spiders don't bother me too much, unless they're particularly creepy lookin', but the way spiders in games relentlessly advance on you with their creepy quick spider walk is just... Ergh it doesn't bear thinking about.

Fog, darkness...anything that obscures my vision (thanks, Silent Hill).

Being chased (Amnesia, I'm looking at you, you prick).

Sharks and bears. Any water level with sharks I will scream like a girl and drop the controller (or obsessively pause he game). The bears in Skyrim I'll run from going "EEEEEEEK!".

Sudden loudness. I'll be sneaking around when suddenly, "HEY! sSDJFSDFLJDSFLSDFJLDSFK!" - and I jump and get this burst of unneeded adranaline.

Air vents and deep water, respectively due to Dead Space and THAT GODDAMN EEL in Super Mario 64 (I was young and it scared the crap out of me at the time, don't judge me.) Actually, the water thing kind of leaked out into my regular life as well, I get nervous swimming in deep ocean. There's just something indescribably creepy about knowing that there are unknown creatures somewhere under the surface that are thouroughly at home in one of the few remaining earthly environments in which humans are largely helpless. 'Ya know?

It's not really a phobia since I don't irrationally cower in fear of it, but in any game that has a section with deep water (i.e. can't see shit in it), I'm always paranoid about sharks even if I know there's no sharks in the game. Pretty sure this paranoia came from Resident Evil. Damn my brother for letting me play it as a six year old.

For me, it is the sound of air raid sirens. Silent Hill gives me the willies, BIG time. Another thing would be things that aren't quite human, such as redeads and headcrab zombies. Urgh. I would say the thing that gives me the biggest fear in games is being unarmed or not being able to defend myself. Playing through the Project Zero games was an absolute nightmare. For my fiance, it's spiders, deep water, the dark and Andross from the first game. Urgh.

Things that are invisible and are hard to kill, or things that come out of nowhere, Dead space and STALKER for example they freak my pants off and I flip off my chair, ugh...so scary.

All kinds of killer fish in games scare me. This stemmed from the sharks in the original half life game and has stemmed a phobia of water mammals in real life. Thanks half life

turret sections. (the kind where you man them, not destroy them.) the lack of mobility and forced ranged gameplay make me nervous for my character's life, considering i'm all about running around and doing melee attacks.

I think i speak for everyone about this one, its Stuff Which Could be Lost Forever....like in Final Fantasy 7 when you get to the

scrapyard somewhere, theres a reactor of somesort, you can either choose to stick your hand inside, or not, if you do stick your hand in, you get the Titan Materia...dunno what would happen if you do stick your hand in other than that, so without reading a guide, i avoided sticking my hand in, thinking it was like that thing with the nest.

By the time i discovered what i missed, i simply ragequitted...think i got up to Wutai. :/

Definitely FPS chases. I remember this one scene in Condemned 2 where you're chased by a giant bear. That one still gives me the chills.

I also hate it when you're placed into a situation where you have no means of self defense, like the opening of Dead Space 2. As soon as I could melee with the flashlight, I felt safe and okay.

Also, pretty rarely, but sometimes I get scared by water. In real life, I never swim in places I don't know and it drives the missus crazy. Pools and stuff are cool, but dark, unknown water? Hell no, could be sharks in there. Okay, so it's inland and in the cold north, but still. And this, well whaddya know, transfers into games. Unless it's the kind of game where I can be at least a bit deadly underwater. Then it's cool.

Zombie-type enemies (i.e. The Flood from the Halo series) and weapon-/weak point-specific enemies that charge at you and can cause you to die instantly or with a few hits.

Amnesia and Resident Evil 4 are NOT for you my friend :p
For me it has to be instant kills, I get extremely paranoid about them.
Yep, I was one-hit decapitated by a Sliver Hand leader at the end of the dungeon with no saving.

Needless to say, when I reached him again, I slowly picked away at his health with a bow from the shadows.

Fear of heights. Levels with high risks of falling are uncomfortable for me, i'm usually trying to be very carefull in such levels. The experience of falling in almost any first person game makes me feel really uncomfortable ( i unconciously hold my breath and my legs go numb).

Enemies that can't be killed, look terrifying as hell and kill you almost instantly.
REALLY hate (yet adore) Amnesia for this. I can never go back to that Choir again... Creepy screech going on in my ears, running to the door with my merry life, turn to the side.

and then I scream and die.

And the first time facing Death in Persona 3. The jingling chains are nightmarish.
Oh god. I remember how afraid I was when that happened xD That, mixed in with Fuuka going all "RUN OR YOU DIE!!!11!". After that I fled for the nearest exit whenever Fuuka could sense Death... until New Game+ where I got revenge.

Also, jump scares, intended or not. Things like getting hit by something (for example an arrow, a bullet, a spell) when I'm not expecting it. I literally jump out of my chair whenever this happens. The sudden Skeever behind me biting me in Skyrim, or the random well-hidden sniper in Battlefield 3 is a criminal for attempting to take my life through provoking a heart-attack... xD

Things that are humanoid yet move in twisted, weird ways and make clearly inhuman noises. The Silent Hill nurses are a good example of this. Redeads, Gibdos, and other stuff like that.
Especially if they make a really loud noise when they discover you or something similar to that, and causing you to be unable to take any actions until you mash enough or something to that extent.

A sudden change in tone, like combat music suddenly starting out of silence as an enemy hits you from behind. There were many times in Skyrim where a bear would somehow get behind me and not make a single sound until after he took a swing, leaving me to jump about 3 feet in the air and clicking my mouse as quickly as possible.

Water. There's always SOMETHING in the water. It doesn't matter what, I just know that 9/10, it's not friendly. This happened so much that it almost became a real-life phobia for me. And even then, the breath meter spooks me. I don't perform well under time pressure.

Old school Ice levels, especially in side scrollers where you jump and tap left right left right left right to not slide off some impossible platform. I'm looking at you "Iceman" (megaman series)

Saving immediately after something very bad happens. You know that horrible feeling when immediately after you load a game you get killed, or find that you've used something that you really need ten minutes later? I don't need the game to keep all of my quick-saves (like Far Cry 2 did, for some reason), two would be enough, but without that, I'm always looking for a manual save before I commit to something which seems like it might be regrettable in hindsight.
